Authorization

I have class tomorrow, and DH has to go out of town to help his elderly parents get ready to move.

We have friends who will be taking care of DS tonight and all day tomorrow.

I need to come up with a quick sheet to give them with the important information to authorize treatment if neccessary.

I'm going to google it to see what I can come up with, but does anyone have something they can share?
