Maternity Leave paper work

I called my doctor's office today to find out if I could just fax them all of the paper work and then they fax it back to me.. they said no I would have to bring it it.. which is fine, no big deal. The part that bothers me is they are going to charge me per page to fill it all out. It is $1.00 per page for the first 4 and $5.00 for each page after that.

Anyone else have to do this? I think it is ridiculous!

  • Not sure yet. This is on my list to ask about at the next appointment. Any chance they will waive the fee if you just ask them?

  • No, and I have to give the money with the paper work or they will not fill it out. It isn't that it is expensive or anything just the whole idea that I have to pay for something like this.

  • When I had DS my Drs office filled out my FMLA paperwork for no charge.  They do have a policy that they will fill out one form for free and then subsequent forms are $5 per form.  I don't think they do per page.  I think I remember it only being 2-3 pages so hopefully it will only be about $3 for you!  Still sucks though!

  • My doctors office has a sign saying they charge for filling out forms, but they didn't say anything to me about it when they filled out my initial disability forms or the extension form.

  • Maybe I will just take it in and see what they say, not offer the money but if they ask for it give it to them. I just don't understand how they can charge to fill out paper they should be doing anyway. It really bothers me if you can tell. I know $5.00 isn't much money but, it is still crazy!

  • ha that's cheap.  It is going to cost my $75 to have my maternity leave paperwork filled out; that is more than I pay for a doctors visit. Oh well, it is worth it to stay home for as long as I can!
  • Mine was $20. I thought it was really weird that I had to pay to get them to fill out papers!
  • Yep!  I have to pay $20 so I am waiting to get my hubby's FMLA paperwork too hoping they will only charge us one fee for both forms or maybe they can just fill out one and we give a copy to the other employer????  Glad I am typing this... I am giving myself good ideas. :)
