Babies: 6 - 9 Months

Stupid question regarding medicine

DS has a cold and I just bought some tylenol for him. It is different than the previous version we had. That means the dosage is different. Previously I had been giving him 0.8 ml of 80mg of Acetaminophen - NOW I have 160mg version of Acetaminophen. DS's pediatrician gave us a chart for how much he should be getting but I'm a little confused. According to the chart he should be getting 3.75 ml. Does this should right or am I confusing the dosages? He weighs 19 lbs and is 8 months old.

Re: Stupid question regarding medicine

  • 160mg/5ml bottle?

So the dose of tylenol is 10-15mg/kg. your son is 19lbs, or 8.6kg. (1lb=2.2kg)

Per my calculations he should be getting 2.8ml of the 160mg/5ml tylenol, or 90mg.  ( at 10mg/kg)

at using 15mg/kg he'd get 4.0ml, so yea, its right in line with being correct.

Anywhere between 2.8-4ml would be in line with the proper dose.

I can't set up my nice ratio on here to show you how I figured it out, but 160mg/5ml = 90mg/xml then divide!

  • Not a stupid question!

Let's walk through it. Previous concentration was 80 mg/ml and you were giving 0.8 ml. So, the dose you were giving was 80 mg/ml x 0.8 ml = 64 mg.

Your new concentration is 160 mg/5 ml (double check but I'm pretty sure this is the concentration of children's Tylenol, and the 80 mg/ml is the infant concentration). So, if you were to give 3.75 ml, you would be giving a dose of 160 mg/5 ml x 3.75 ml = 120 mg, which is almost twice as much as what you were giving before.

Dosing of tylenol for pediatrics should not exceed 65 mg/kg/day, divided in 4-6 doses (roughly 10-16 mg/kg/dose). Your kid weighs 8.6 kg, so each dose should be between 86-138 mg, and maximum 560 mg/day.

So, take home message is that you were probably underdosing with the previous concentration you had, and yes, 3.75 ml of the new concentration is fine and probably will work better than what you were doing before.

  • That does sounds exactly right.  If you want a quick guide to dosing, this is from Dr. Sears (https://www.askdrsears.com/topics/childhood-illnesses/medicine-cabinet/acetaminophen )

    ACETAMINOPHEN DOSING CHART

Dose is every 4-6 hours. When treating fever or pain, I like to use a "strong but safe" dose so your child will feel better sooner. I have designed this chart with narrow weight ranges so you can give the best dose. Dosing chart for suppositories is below regular chart.
