Lovenox dosage question

I am on 30mg once a day of Lovenox and they told me to stop the baby aspirin. Today I've started researching this online and it seems most are on 40mg a day and still do the aspirin. I called my doctor's office and talked to a nurse and she said what I'm on is fine. I'm not convinced. I have APA and am homozygous MTHFR. Would you push for more or to talk to to the doctor yourself about it? 

Also what are you on?

  • I'm on 40 mg of Lovenox.  I was on baby aspirin.  They had me stop it.

  • OB. I asked for a hematologist referral and they told me that they work with a hematologist so that wasn't necessary. Do most of you see hematologists separately though? I almost want to push for it.

  • I don't see a hematologist, but my OB did send me to an MFM due to my clotting disorder.  I take 40mg daily, but I stopped the baby aspirin.

  • Can I ask a really personal question (that you all can not answer if you don't want to) but how much do you weigh? I just read online that they base it on your weight, so like someone said on another board, they might increase it as I gain weight throughout the pregnancy, right?

  • There are 2 types of Lovenox doses - prophylactic and therapeutic

    Prophylactic  - used to prevent m/c and blood clots.  It is TYPICALLY 40 mg once a day and is not weight based.

    Therapeutic - used to treat blood clots.  It is weight based and is usually twice per day (a typical dose would be 80 mg twice a day but would increase as your weight increased throughout the pregnancy.)  

    The reason I asked which kind of doctor was treating you is because I thought maybe your doctor knew something new about prophylactic doses, but since it is just your regular OB I think I would lean towards discussing your dose further with your OB and/or trying to get in to see a MFM or hemo.

    (I really can't speak to the aspirin since I am not on it, but I am glad some others have chimed in to help answer that question for you.)

  • I am on 40mg of lovenox and a daily baby aspirin. I spoke to my doctor yesterday about my positive ANA test, in which she said I will be tested for antiphospholipid antibody syndrome (I'm assuming that is your APA, right?).

    Anyways, she clearly said that the treatment for APA is baby aspirin and lovenox. If I were you I would push to see a specialist.

  • I also have APA. I'm on 50 mg of lovenox twice a day and a baby aspirin. I lost my first pregnancy at 21 weeks on 30 mg twice a day and no aspirin. I see a MFM and hematologist. I would ask about getting a anti-factor Xa test done. That can tell you if you are therapeutic on the current dose of lovenox. I was getting nervous b/c I started out at 70 mg (considered a weight based therapeutic dose) but my factor Xa was really high so they had to lower it down to 50.  If you don't see a MFM doctor I would really consider it. I see them about every 2 weeks with U/S every 4 weeks to monitor growth. Good luck to you!
