Had our A/S today, any experience with fetal hydronephrosis?

(Also posted on the Feb. 2012 board)

The baby is still a boy and everything looked great.

But my doctor did say that the baby has very slight dilation of one of his kidneys and ureter. She said it is called fetal hydronephrosis. She said that they see it commonly in baby boys and most "grow out of it". She said it's nothing to be concerned about and that at my 32 week ultrasound they will check it out.

So stupid me goes home feeling good about everything until I google fetal hydronephrosis.

I am so freaked out now!

We had an NT scan and all the blood work from all the appts. and everything came back great. Everything has always been normal. My doctor even said it was very slight and not to worry...but after reading online...which you should never do...I am scared.

Does anyone have experience with fetal hydronephrosis, or know of anyone who has?

  • One of our March Mamas had the same question/issue.  Here was my response.  Her Doc's concern was that it was an indicator for Downs but if all your other blood work came back ok and you have no other indicators my below responses should help you understand what pre-natal hydronephrosis is and what it can mean.

So pre-natal hydronephrosis can be an indicator of a lot of things, including vesicoureteral reflux (or VUR which you basically alluded to in your post and requires more than just a few months of antibiotics but rather years of daily prophylaxis and if not treated soon and by a pediatric urologist can cause signficant scarring in young kidneys).  Hydronephrosis also accounts for about half of all prenatal defects.  Typically it is indicative of a kink or stricture in the ureter that causes backflow into the kidneys (called a ureteropelvic junction [UPJ] obstruction). If you want to chat more about VUR please feel free to page me.  My daughter has it and has been treated with antibiotic prophylaxis since she was 3 months old (she is almost 3 years old now). I strongly suggest that you chat with your OB about the possibility that this is VUR or UPJ and not necessarily downs (considering your other testing).

FYI, my DD (the one with VUR) had perfect kidneys and bladder on U/S and was later dx with VUR after two very serious UTI at age 2 months.

  • I think that's what DS had.  They called it dilated renal pelvis.  I had a couple extra u/s to keep an eye on it and we had to see a urologist 2x after he was born - 1st when he was about a week old and then when he was 5 mos old to recheck.  they did u/s's of his kidneys and it resolved on it's own.  It was super scary news to get, but the peri told us it was good to know.  In the past, a lot of parents didn't know there was an issue until their child was in kidney failure.  DS's was pretty bad, though.  At 18 weeks, his renal pelvis was bigger than the size it should have been at birth.  If it doesn't resolve before birth, make sure you mention it to your pediatrician.

  • My son (who is now 2) had this.  I was worried, but my doctor said the same as yours--that it almost always resolves on its own and they grow out of it.  And he did.  No problems as of the 36 week ultrasound and he is fine now!
