yes I still have the head support that came with the car seat on. It is so thin and it doesn't seem to bother her. I hadn't even thought about taking it out.
We took the infant insert out but left the head support in. I'll only take it out once his head gets bigger and seem squished.

DS will be 6 months next week (and he's 20 lbs already) and yes, we still have the head support in. We probably can take it out now though. He wasn't ready for it to be removed when we took the rest of the insert out.

No, I just took it out a week ago. DD is starting to always want to sit up and pushes herself forward. With the padding in she would really push against it and look uncomfortable in there with it. Plus, I think she has a big head. (sorry to my baby but I felt like I was squishing her head in there).
We just took ours out last week. I didn't realize how much it was restricting their movement until I did. Now they look all around and seem to enjoy more of the scenery.
I removed it a couple of weeks ago when I noticed that his ear was always bent with it in there. I can't decide if the bent ear is worse or the lack of support for his neck when he falls asleep in the seat...

I took it out when the harness straps had to be moved up to the top slots. Here is what the manual says about the head insert
NEVER use head support with harness in upper slots.
Head support could interfere with harness adjustment.
It is ten or eleven pounds. We stopped using ours when M was a month old or maybe 6 weeks.
You're supposed to take out the infant insert at 11lbs, not the head support.
OP, I just took out the head support yesterday.
