Food Allergy

GI doc? Allergist? ENT? Help!

Hi you guys, 

My GI doc is worthless and hasn't returned my calls. Little one has been on the Neocate for 2 weeks, improved drastically and then after about 10 days starting screaming/arching/not eating again.  He eats 2 ounces and then quits, maybe 3 ounces if he's really hungry.  He's never taken more than a 4 oz bottle in his life and he's a beast - 13 pounds, 25 inches long, 10 weeks old.  His daily intake is around 20-24 oz a day, usually around 22oz.  

I obviously need another doctor who will return my calls, but should I stick with the GI docs?  Try an allergist even though he's only 10 weeks old?  Maybe an ENT to scope him and check esophagus??  

He reacted negatively to prevacid so we stopped that.  Pedi put him back on Zantac but he's only had two doses so far, so no idea if that will help or not.

  • The key to reflux is small frequent meals. My daughter was unable to tolerate more than 3 oz at a time until she was probably 6 mos old. If he is only eating 4, he may be telling you that is all he can handle. Since he has reflux, your best bet is to stay with a GI. If you aren't happy with your current dr, get a new one. An ENT can only look at the airway side of the throat, so feeding/eating/reflux issues need to be seen by a ped GI. If he had a negative reaction to Prevacid, you can try a different PPI like Prilosec or Nexium. It sounds to me as if his reflux is not well controlled. Keep in mind that any medication can take up to a month to see results - 2-3 weeks for the medicine to kick in and an additional few weeks to heal any damage in the Esophogas. It is best to make one change at a time and stick with it for at least a month to see if it is working.

  • Thanks for the advice.  I called my pediatrican today and she prescribed Prilosec.  It's at a compounding pharmacy and they take a little longer, so he should start that tomorrow. Will do the Zantac in the meantime.  I made an appointment with my ENT, who I love and who also specializes in pediatrics, after chatting with his nurse earlier today.  We all agreed it was at least worth a visit.  At this point, I figure it can't hurt to have one more expert look him over, but I agree that a GI doc is our greatest asset right now.  Mine finally called this evening and left me just as confused as I was before.  He said that after two weeks on Neocate he's probably as good as he's going to get, and yet everyone I've read about on this forum says they needed 3 or 4 weeks for best results.  So when I said, "Well if this is as good as it gets, then it's not good enough - what next?"  he didn't really have any great advice.  Just to challenge with breastmilk FIRST for 4 or 5 days and THEN star the Prilosec.  My instinct tells me to do the exact opposite!!  We'll do Prilosec for a few days and if he doesn't react poorly, then I will challenge with breastmilk...and find a new GI!!!!!!!!

      

  • A quick note on the allergist thingy, I don't think that 10 weeks is too young to see an allergist for your little on. My little boy had a nut allergy at the age of 10 weeks old that I would not have found out about had I not gone to an allergist. Its always a good thing to cover your bases and trust your instincts.
