I would spelled it Ciara upon hearing it (because I love Irish names).
I would pronounce it the way MJ says, but even though they sound the same, Chiara and Ciara are two radically different names to me because of the ethnicities each invokes.

I would know how to spell it, but I'm part Italian and Chiara is my favorite Italian girl's name.
For what it's worth, DD's name is something most people wouldn't know how to spell just from hearing it, and I don't think it's a big deal. Even a lot of familiar names have more than one spelling, so if, say, I tell people my name is Juliet, they will frequently write it as Juliette (or something else; I've seen all kinds of, um, creative variations). For me that's not a dealbreaker, and I would prefer to use the correct spelling in the case of a name I'm using because of my/DH's ethnic background.
