WWYD: Nursing and medication?

So my mastitis has not responded well to antibiotics. I've already been prescribed 2 different ones (dicloxacilin and keflex) with no results. Today the doctor prescribed me a third antibiotic (the one you get when all else fails - bactrim). Unfortunately it isn't safe for nurslings under two months of age, which means that I would need to stop nursing K, and I would need to take this medication for 10 days.I'm concerned with K being so young that 10 days straight on a bottle will destroy our nursing relationship. I think my fever may have finally broken this afternoon but my breast is still very red over a large area. I'm considering staying on the current course of antibiotics (keflex) and see if I continue to improve, but I run the risk of getting much sicker if it doesn't work :-( Should I stop nursing and take the new antibiotic, or stick it out a little longer and see if the keflex keeps working so I don't ruin my nursing relationship? For what it's worth, my fever hadn't yet broken when I saw the doctor earlier today. I tried emailing him to see what he thinks but haven't heard back yet... I may DD later...

 ETA: Of course if my fever returns or the redness continues to grow I will start the new medication ASAP

  • I don't know what I would do because I don't know what is safe. If my dr thought it was safe to keep on with what I was doing I would try it, but without consulting with my Dr I don't know. I am sorry for not being much help. I do hope you feel better soon, mastitis sucks!

  • I'd ask the advice of a good LC.  I take a *** ton of medication and I can't tell you how much stuff I've been told was a no-go while nursing and when the rubber hit the road not once was a single thing actually harmful.   The only time I actually pumped and dumped was undergoing a test that involved injection by radioactive material and even then instead of dumping they took my breastmilk and tested it at 12 hours, 24 hours and 36 hours and not once single trace of crap was in the BM (got it all written up in a journal and all Stick out tongue)
  • I've been on the Keflex for 52 hours now.... My fever has been gone for about 5 hours and I haven't taken any analgesics since 8 this morning, so that's a nice improvement. I didn't even think about calling an LC. I think I'll put a call into one tomorrow morning to get their take on the safety...

  • I think you can definitely give the Keflex a little more time, if you are seeing some improvement, that  is not a very long time. If you see worsening as you said you can change to bactrim. I agree with above, that the risk is not very high with a full term, non jaundiced one month old baby, and you  could probably continue nursing but that would require close monitoring of the baby by a doctor, maybe you want to talk to the pediatrician on Monday. The risk is a lot higher in preterm or jaundiced babies.

  • Are they concerned at all about an abscess?  That seems an awfully long time to go without improvement.  The second time I had mastitis I didn't respond to the antibiotic and within about 36 hours my OB was very concerned and I had to see a breast specialist and have an imaging ultrasound of the breast tissue.  I literally had to see the breast specialist every day until she was certain I was in the clear (every visit she would draw on my breast with a surgical marker so we could make sure the redness wasn't increasing).

  • There was some concern about an abscess, as well as the infection being MRSA, but they didn't run any tests to confirm either. I think that if I have to not nurse for 10 days they better darn be sure that that was the only course of action. Today I'm starting to feel better, and the redness has subsided a little bit, though not as much as I or my DH would like. I have a doctors appointment first thing monday morning, so I will talk to him about possible ultrasound then, and monitoring of K if I need to take the bactrim. Thanks for all the input.
