That is so sad. I can't even imagine their pain. And the poor employee who was helping must feel horrible too.
Not long after Alex was born, Wegmans got new shopping carts and the infant seat didn't fit in the top part anymore. It prompted me to buy a sling (which was recalled 2 weeks later). I think that more places are purposely making them not fit so people won't put the infant carriers there. It makes me nervous seeing the seats teetering there.
How awful.. especially that it was a store employee pushing the cart.
Something similar actually happened to me when Eve was a baby. I had her carseat in a small cart where it didn't really clip in well and we were walking through the parking lot and and i hit a pothole or something with the cart and the carseat flipped over. thank fully it went backwards INTO the cart instead of out of it.. and she was strapped in tight.. (i rarely loosen the straps when i take them out of the car) so she was fine. But it was scary!
That being said, I still put the carseat in carts all sorts of ways I shouldn't.. Lately if I'm using a smaller cart, i put Eve up front and put Natalie crosswise across the top of the big part..shes not clipped in or anything.. the seat is just sort of balancing there. But, IMO, its much safer than risking Eve being in the big part of the cart and toppling out.

So sad...Now I usually put Andrew in the large part of the cart and Evan in the seat, but when it was just Evan, he went up on top. I try to go major grocery shopping by myself now, so this arrangement usually works, and helps me spend less at Target, because I can't fit much...

I'm kind of confused - did the baby in the article fall out of the carseat as well? If the baby was strapped in and the handlebar was up, I'm confused how the baby was injured. Not criticizing, just wondering.
I never trusted putting the carseat up on the top of the cart and I was told not to do it, but I don't remember the source or anything. I could never get the bottom of the carseat to click/clip on, so I just couldn't do it.

I did a few times until Wegmans got new carts and his seat didn't fit well at all. I would pile groceries in the top part, underneith and in the basket around him. Then I switched to a sling (and two weeks later the sling was recalled because babies died in it). I ended up getting a shopping cart cover, belting him in and then propping him up with packages of diapers on either side until he could sit on his own.
ETA - Don't feel like a bad mom at all. There are so many people who do it and in the past the seats were made to fit in the carts. When we know better, we do better.
We always put them in the top too...unless it REALLY didn't fit. DH insisted, though that we always keep the handle out (like not fold it back as you would to take the baby out)...just in case. God I love my super safety freak of a DH...even though at the time I rolled my eyes at him...thinking there's no way the seat would fly out of the cart.
I feel like printing this story out and taping it to all shopping carts.
