I think this varies from hospital to hospital because I've seen two answers to this question, and my own experience. With DS, I'm almost positive they didn't pull it until after he was born because I remember wondering if it was going to hurt (it didn't)...and I also pushed for 3.25 hours, so I'm not sure how I would have made it that long. However, I've also seen many responses on TB that say they had it removed prior to pushing.
They took mine out just before I started pushing. My OB said it just gets in the way otherwise. I did still have my epi on at this point though. The epi was removed and stopped after my OB finished stitching me up from my episiotomy.

Mine was removed after I delivered and was transferred to a recovery room. I was also worried it would hurt, but I didn't feel a thing (and the epi had completely worn off by that time).
During this pregnancy I've had a catheter and coming out didn't hurt near as bad as putting it in (which with an epi obviously you wouldn't feel it)... but I was just curious if I would be pushing with this tube coming out down there.

I had a c/s, and they took it out 2 days after the surgery. They tried to take it out the next day so I could urinate on my own, but I had a "lazy bladder" and literally couldn't pee at all despite how hard I tried. I had the urge to pee so bad but just couldn't, so at one point I begged them to put it back in, and they finally conceded. I ended up filling up a huge basin (sorry, tmi)! It took a few days for my bladder to get back to normal. I don't know if that is how it goes with a vaginal birth...

It might depend on the hospital. Mine doesn't leave it in the whole time - just puts it in to empty the bladder and removes it.
