? for moms of twins

Random question for all of the moms of twins-- my good friend just found out she is 10 weeks pg with twins. Her OB said that she recommends a c-section for all moms of twins regardless. She was wondering how common practice this is and was hoping I could get some feedback about vaginal deliveries (did you attempt on? have a successful one? pre-term labor? etc) as compared to c-sections. I also posted this on my local board knowing that some of the answers may vary based on region but and they suggested I XP here. TIA!

  • Stastically, the rate of c/s vs vaginal for twins is around 50/50.

    I attempted for one, but my boys had other ideas. :)

  • I would find a new OB ASAP...unless she is okay with a c-section. One of the first things I asked my OB was if he was okay with me delivering vaginally, AND if he was comfortable delivering a breech baby (in the case that baby B flips because I didn't want the "double whammy").The only "required c-section" that my doctor performed for twins was if Baby A was not head-down. And I think that is typical of all twin pregnancies if Baby A isn't head-down.

    I delivered both vaginally and it was a piece of cake after I got my epidural. Pretty much when baby A came out, the nurses just placed their hands firmly on each side of my stomach to make a "pathway" for baby B to come down without flipping. 10 minutes later she came out.

  • I believe a lot of OBs "recommend" c-sections as overall they are a less risk for both mom and babies.  All OBs in my practice recommended this to us, but I explained that I wanted to try a natural birth, and they were all okay with it, assuming that the boys were in position... they weren't going to allow the boys to be in any danger and I understood that.

    At the end, we ended up with a c-section bc I become pre-E.

  • Both of my babies were vertex, I delivered them vaginally. My practice was comfortable doing a breech extraction if necessary or if baby B turned during delivery. The practice prefers to do vaginal deliveries (even of twins) over c/s's and never indicated that I didn't have a choice in the matter.
  • I had a c/section with DD1 but was a good candidate for VBAC even with twins. I did not attempt it (I had an elective repeat c/section combo package with a tubal). However, if I had gone into labor on my own before my scheduled date both my docs and I were comfortable with VBAC.

    I read somewhere that if you want to know your chances of "ending up" with a c section look at your docs c/section rate and you'll get your answer. If your friend wants a chance at a vag. delivery, I suggest she finds a new OB asap. I think too many docs push their comfort zone, scheduling preferences, etc on people instead of facilitating or advocating for trying to achieve the type of delivery the patient wants.

  • My OB said we could do vaginal delivery if Baby A was head down, and then if Baby B wasn't then try to do an adversion (is that the right word) to get them head down. If he wouldn't turn then he would only do a c/s no breech extraction.

    In the end, A was breach and B was transverse so my only choice was a c/s.

  • My OB will do a vaginal delivery 1. both are head down 2. A is head down and B is breach but has to be SMALLER than baby A.  He will do a C-section if 1. Both are head up 2. Baby B is bigger than baby A.  A always comes out first (A is the one that is lower, it always stays that way) and so if the B is bigger then you run of risk of the baby not fitting through the birth canal so he likes to have baby B smaller than A.

  • I would also recommend looking for a new OB. Mine preferred vaginal deliveries as long as baby A was head down, and I ended up with a breech extraction for baby B with no problems and no complications at all.

  • Given the positions of the babies are acceptable, it should be up to the mother.  I chose a c-section and both my babies were head down.  I felt it was the safest option for me and my babies given my past abdominal surgeries and family history of difficult births.  It was the right choice for me, but I'd want an OB that was supportive of me no matter what delivery I chose (or chose me).

  • I had a scheduled c-section because both babies were breech.  However, my doctor was comfortable with trying a vaginal delivery with a head down Baby A, although she was not extremely comfortable with a breech extraction.  When I first brought up a scheduled c/s my Baby A was head down and the doctor didn't automatically say "okay, let's do this".  It was only when they both turned breech and stayed that way that we set that as the birth plan.

    If I had a doctor pressuring me into a c-section at 10 weeks PG I'd find a new doctor.  That doesn't sound like someone that is up on the latest practices as far as twin delivery.

  • My OB also requires section for twins.  I had a repeat section with Landry and Hudson and wouldn't have wanted it any other way.

  • I was induced at 38w1d.  Baby A was head down and Baby B was breech. My doctor said as long as Baby A was head down, I could attempt a vaginal delivery. Not once had my doctor discussed a C-section.  Baby B ended up flipping after A was delivered and I successfully delivered both vaginally.   I had prepared myself for a c-section just in case, but I'm glad I was able to delivery vaginally.

  • I had a vaginal delivery. I only would have had a csection if baby A was breech.

  • I would never go to an OB that required all twin births to be c-section..they are over used in the US. I fought to have them vaginaly but baby b was stuck in my ribcage so I had no choice in the end. There are so many more complications with c-sections I would never be okay with opt-ing into one unless I had no other choice!

  • The others have pretty much covered it. The other thing that sometimes determines a c section is twin type. (How many placentas?)

  • My OB would have let me try for a vaginal birth if both babies had been head down. Unfortunately my Baby A decided to sit down in my hip and make herself comfortable so I had a planned c-section. My Baby B was flipping up until delivery and was head down when I delivered but it didn't matter because of his sister's positioning.tell her not to get discouraged yet. A vaginal birth is totally possible and many of the MoMs here have done it. Also remind her that it is only a suggestion by the OB. She can't be forced into a c-section. If she does start to feel pressured by her OB, tell her to look for another. Many OBs will let MoMs attempt a vaginal delivery as long as baby A is head down and will do a breech extraction of Baby B if needed.Good luck to your friend and good luck on the remainder of your pregnancy!

    My MFM doesn't like to deliver mo/di twins (one placenta, identical twins) vaginally.  Too much risk for the placenta separating from the wall after Baby A is born, but before Baby B is born.  Therefore, I had a c-section.

    His requirement for a vaginal delivery was that both babies are head down, and that there are two placentas.

  • My OB said that she recommended trying for a vaginal birth with twins if Baby A was in the correct position. Both of my babies were breech, so a c/s was a must for me, but otherwise, I could have done it. 
  • I was told that as long as mine were both head down, a vaginal birth would be preferable. I ended up with a c-section due to severe pre-eclampsia, but we were planning on a vaginal delivery because both had been head down since 20 weeks. It's strange that her OB would completely rule out the possibility.

    Ditto this. I delivered both of my babies vaginally. Baby B did flip to breech (he flipped back and forth the whole third tri) so he was delivered by breech extraction.

    I'm curious; do you know if he had the same policy for fused placentas? Mine fused (and I delivered both babies vaginally) but I didn't know they had fused until after the placentas were delivered and the nurse told me there was only one. (I had been told at my first u/s at 12w that there were two so I knew they must have fused. That and my boys are clearly not ID. ;))

  • I'm in Canada (I have no idea if it makes a difference) I would imagine it would be different for every OB and their comfort level.

    My OB is very pro-vag delivery. I wasn't given an option b/w vag vs. c-section. He said as long as Baby A was head down then I was having a vag delivery and there was no reason to have unnecessary surgery (His words not mine) He's both skilled and experienced at breech deliveries and I felt comfortable with his care.

    This. I delivered my twins vaginally with no problems after being induced at 38w1d.  I was 10cm at 1:30 pm and both were born before 3:00 pm.  :)

  • If all would have been well, I would have attempted a vaginal delivery. My OB will deliver twins vaginally if both are head down and Baby A is larger than Baby B. 

    I had one breech baby and a huge fibroid blocking the exit, so automatic csection for me.

  • It's actually more common than people think...I would have done it if they weren't early and one wasn't breeched.

  • I delivered our twins vaginally, after two previous vaginal deliveries.  My OB was very pro-vaginal delivery : ) Both our girls were head-down, but both came out face-up. It was still a much easier delivery than my much bigger singleton boys.

  • I only had one because my baby A was breech.  I would switch docs if they suggested c-section for all twins.
