Halo fleece ones are good for nighttime. I also found a First Years one at a consignment sale which is AWESOME but it's not warm enough for nighttime. The Summer Infant ones don't stay on IME.
We use Aden & Anais blankets and do the Escape Proof swaddle from You Tube. She can escape from the SwaddleMe blankets.
This is us too. He can get out of the swaddleme blankets in no time.

My LO escapes almost all swaddles, but for some reason for naps he will stay in the swaddleme and not try to break out. At night time no way, he would be out of it in 2 min, we have to use the halo swaddle sleepsack at night.

I just use the carters ones typically and I have not had a problem with her escaping. I also have a Miricle blanket which does the trick, only I find it a little hard to get her in it.

We've nicknamed my son Houdini b/c he escapes almost all swaddles -- seriously! However, we've had the most luck with the Summer Infant swaddles. All that velcro does a pretty good job at keeping his hands by his sides.

I love my Woombie. When I used the SwaddleMe or a regular blanket, DS would wake up at night and start screaming when he couldn't move around. The Woomie is stretchy enough to let him wiggle a little bit, but still constrictive enough to keep him from whacking himself in the face and waking himself up.
We use the Swaddle Pod from Summer Infant which is just like a woombie but half the price. It's not very warm though so we zip DD up in a Halo Sleep Sack too. Lobe the Swaddle Pod though!
I love the blankets from www.Swaddledesigns.com. They're huge and work well as a crib blanket through toddlerhood.
We've tried lots of the swaddlers(SwaddleMe, Swaddlepod, miracle blanket, A&A) and find these, the flannel ones, the best at keeping LO's arms from breaking free and waking him up. We don't bother swaddling his legs though anymore as he usu kicks free in a few min anyway.

we use the summer swaddleme ones, but she has figured out how to make two hand holes for herself near her waste every night. its pretty funny! but it kind of makes the swaddling a bit more ineffective...
naps she just goes without being swaddled, as she doesn't seem to like being swaddled at all. doing so at night keeps her from waking up due to reflexes.
SwaddleMe was a joke for us- LO figured out how to bust out of those the first day, and the 2nd day, and the 3rd day... sigh.
Someone gave us a blanket from Swaddle Designs and we use that for naps bc it won't keep her swaddled all night- she'll usually find a way out of that.
Night time- per other bump recommendations- miracle blanket. Hands down. I'd recommend it 1000 times over. She sleeps 9-10 hrs consistently EBF.
