Share your birth plans, please?

I need to finally make one, something concise.  I don't want it to be anything more than a page, but I do want to encompass a VBAC and if I need a repeat C/S.

My CNM is very much on the same page as I am about everything, but I know I want a birth plan if, for no other reason than to remind MYSELF of what I want when I'm in the throes of labor, ya know?  I regret not having one before for that reason.

    I only made a c/s birth plan but I would be happy to share it with you of course.
