Depends on what is causing the rash. S has been getting it a lot lately, and we thought it was the switch from bf to goat's milk. Turns out it was a heat rash, she just needed air on it (twice a day for up to 1 hr, cleared up by morning). I usually just use either vaseline or zinc oxide (mostly this) as a barrier. If the rash is a yeast infection or something else, I'd check with your doc, creams may not cut it (at least OTC ones).
Boudreaux butt paste. I swear by it and it doesn't have a nasty smell.
Also, I put it all over the nether regions for every diaper change. I thought diaper rash cream was only for when baby actually had a diaper rash, but early on, the nurse at the pedi's office told me to use it all the time. So that's what I've done since the beginning and so far, DS has not had any rashes.
Triple paste. Daycare recommended this, and for both of my kids, a diaper rash goes away within 2-3 diapers. My pedi did recommend getting the area very dry between diaper changes, in order to avoid yeast infections.

C's had some diarrhea and really sore bottom for the last couple of days.. I think it's those dammn back molars. Aired him out for a while yesterday and that seemed to help a little. Tried to give him a soothing bath last night and he just screamed in pain. Poor little monkey.
I use Max strength Desitin and it keeps it in check, but isn't going away. If it's still bad today, I'll switch to Boudrouxs.

triple paste. hospital made up a home-made version for a neighbor when her kid's bottom looked like ground meat with maalox, corn starch and zinc oxide. I prefer less work by buying the premade stuff and that always worked. I will also say I liked boudreaux's but it made DS break out in a horrible rash so I've stuck with the triple paste for regular changes too.

I'm a huge fan of Triple Paste. I use it at most diaper changes (unless DS is too squirmy) and he's been rash free for a while. I also use Desitin.
For us, Zinc Oxide is key.

No ointment at all. And, we've used every.single.one.
Since using Caldesene Zinc Oxide Talc, R has never had a diaper rash. Of course one has to be very careful when applying because it can be inhaled.
