VBAC

would you consider switching OBs at 24 weeks??? (longish)

1.  i really want a chance to try a vbac (ds was frank breach - discovered at 37w5d - no time to turn him, too late for a version)
 
2.  i like my ob practice just fine, have nothing negative to say, but i'm not 'in love' with them ... and they have a few conservative policy restrictions that are based on liability, cost and convenience rather than science or current ACOG recommendations:
a. will not let AMA women go past 41 weeks
b. will not induce for vbac
so if i don't go into labor on my own by 41 weeks, i will have a RCS ... no chance for vbac
 
3.  chances are relatively high that i will go into labor on my own before 41 weeks ... which means the policy restrictions would not apply
a.  there are things i can do to increase the chances that i will go into labor (the place i did fertility acupuncture has a 'labor induction tx plan' that includes acupuncture, acupressure and massage ... beginning at 36/37 weeks)
 
4.  devils advocate:   at my advanced age (42), even without a previous c/s my chances of a c/s high ... and my chances of actually succeeding at a vbac are lower than that of a 22 year old ... so even if i were to go into labor on my own before 41 weeks, i still may end up with a c/s. 
 
5.  even if i fail, i will be really disappointed if i don't get a chance to try a vbac
 
6.  there are other ob's in NYC who do not impose the same restrictions and who would take me at 24 weeks
 
so ... would you consider switching OBs at 24 weeks???  dh thinks there is no downside to exploring this option - but i'm wondering if the commotion would be worth it.

  • I don't think it would hurt to look into your options.  Maybe you can call around and see if there are any OBs or MWs who you could see for a short consultation.  I thought about switching during my first pregnancy and I wish I had at least looked into it.

    That said, I don't think those restrictions are unreasonable.

  • thanks ... in general, i don't think the restrictions are entirely unreasonable either ... except that they could prevent me from even attempting to vbac, and that is really important to me.  i had a really hard time (emotionally) accepting my c/s ... since my son was breach and the c/s was scheduled, i didn't experience labor at all.  of course some people think i'm crazy for wanting to feel contractions and pain (and i'll accept that : )

     

  • I agree with your husband, it really wouldn't hurt to at least look into a new group. 24 weeks is not too late at all. I went to 41 weeks and past 41 weeks with both of my sons, so having the option to wait a full 42 weeks would be an important factor for me. And since you didn't go into labor with your first son, you might need that extra week, right?

  • I just posted above you as I am in the same situation but further along.  I am also AMA and getting pressured from my practice about a RCS.

    I think if I were you I would ask around and find someone who is really supportive of VBAC's and set up a meeting.  It can't hurt to keep your options open.  And if you really feel uncomfortable with your doctors restrictions you should trust your instincts.
