Cervix seems short...? for cerclage ladies

I had an appt with the high risk OB yesterday to check my placenta since I've had some bleeding. Since I've had a previous IC with a twin pregnancy he wanted to go ahead and get a meaurement with the transvaginal u/s. My cervix is measuring 2.6cm which seems short? Especially for 13 weeks. He said he is fine with it though? My first cervix check last pregnancy was at 16 weeks and it had funneled down to .5cm and I had a emergency cerclage the next morning. The doc actually had to push back my membranes and put 2 stitches in instead of one. I'm a little nervous this time but both the high risk OB and my regular OB seems to think it will be fine. Apparantly having a previous term pregnancy is working for my favor? Anyway, he did say that the absolute latest they could do a cerclage was 25 weeks and even if I was 5cm dilated they would still try to put one in. I'm hoping my cervix holds...I'm just thinking were starting out kind of short?

  • That is really short especially at 13 weeks I dont understand why they wouldnt put one in now if you already had one previously. It just doesnt make sence. You would think even if your cervix wasnt shrinking they would do a prevenative.

  • And why would they want to wait until your down to nothing to work with and risking your membranes to rupture. The farther along you are in preg poses more risk...
  • I don't know. They are attributing the previous IC to the hormones and body changes with the twins. I have some blood in my uterus too (unrelated) and he did say they didn't want to mess with the cervix right now as long as I was bleeding. I'm not really sure why. We are measuring my cervix again in 2 weeks.

  • I agree that your cervix seems pretty short for only 13 weeks. I lost my triplets to IC at just 19 weeks back in January. My cervix had been measuring consistenly at 3.3cm. However, after a bit of spotting I went in to get things checked out and found out that it was down to 0.9cm. I was admitted to the hospital for a rescue cerclage, but went into preterm labor. They had to try to stop the labor before they could put in the cerclage so I was given medications overnight and was taken back to the OR the next day. Unfortunately, I started bleeding in the OR so they had to scrap the plans for the cerclage and I delivered my tiny babies just hours later. 

    My MFM tried telling me that things like that just happen sometimes with multiples and that they really wouldn't be all that concerned about a singleton pregnancy. The plan was to just measure my cervical length every two weeks and place a cerclage if it started looking short or funneling. Considering the triplets only weighed 680g all together or the same as a 24 week singleton, my husband and I were not comfortable with this plan at all. Going into labor at 24 weeks wouldn't be ideal either. After multiple rounds of fertility treatments, three miscarriages, and the loss of the triplets, we wanted to do everything possible to ensure a long, healthy pregnancy this time around. 

    I got a second opinion from another MFM. I noticed a huge difference immediately. He spent almost 45 minutes explaining all of our options and agreed that our concerns about what happened with the triplets were valid. Because my cervix had changed so dramatically and quickly, he suggested that we would be better off doing a cerclage now when there was less risk of complications. He also said that sometimes, when the cervix starts to shorten, it can change hormones in the body and trigger preterm labor that can complicate the situation even further. Since this is exactly what happened to me last time around, I agreed that doing a cerclage now seemed like a good idea. 

    I just had my Shirodkar cerclage placed a few days ago. Everything is going great! In addition, my new MFM wants to start 17p injections at 16 weeks, monitor my cervical length frequently, and possibly restrict my activity from 20-34 weeks. I'm so thankful that he's willing to be so aggressive and proactive. I don't think my husband and I could handle going through another loss, especially if it was under circumstances similar to what happened with the triplets and there was something that could have been done. 

    I would definitely recommend getting a second opinion from another MFM. Please keep up updated on how things are going. I'll be praying that you have a long healthy pregnancy!
