Cervical length with twins

Hi ladies. I'm hoping some of you can share your cervical lengths at 22-24 weeks gestation if you had it checked and remember. As well, when you delivered your twins (if applicable - if not, how far along you are now) and if it was shortish, what was done, if anything.

Thanks! Everything I read about average/normal cervical length is for singleton pregnancies and I'm not sure if acceptable norms are different for twins.

Re: Cervical length with twins

  • At 22w, my cervical length had gone from over 3 down to 2.8, and they started to get worried. I was immediately put on bedrest but was allowed out of the house occassionally. They wanted it to be at 3 or more at that point in the pregnancy. At 24w, it had gone down closer to 2.5, so I was put on stricter home bedrest.

At 28w, it was done to 2.0, and I was given the steroid shots and admitted to the hospital to do my bedrest there. I didn't have any funneling or contractions. I stayed in the hospital for four weeks until 32w. At that point, it was still at 2.0, so they let me go home and finish bedrest there, as long as I promised not to leave our bedroom except to go to dr's appts.

At 35w, it was still at 2.0. At 36w4d, when I delivered due to high BP, I was dialated to 3-4 cm.

  • I never knew mine with my singleton, but this time around my cervix has been "high, long and closed" which I found out at the MFM last week was 4.32cm.  I think it's alwasy been 4cm or so.  I think they start to worry if it gets around 2.5 or maybe 2 and below?

Can't help on the delivery side of things since I've got a few more weeks to go!

  • At 22 weeks, my cervical length was 4.0 (amazing, right?).  By 24 weeks, it had dropped to 2.7cm, so I was put on bedrest.  That week it dropped closer to 2cm and hung out in that range until delivery at 33 weeks.  The length may have been acceptable for any singleton or multiples pregnancy but that kind of drop in a short time was not.

  • I think you're going to get a lot of varying answers here.  Some women have a strong cervix that can withstand the pressures of twins, others can't.  Mine was a worst case scenario.  I started the pregnancy over 4cm, and gradually got shorter.  Around 19 wks, I was at 2.8-3cm.  When I went back two weeks later, I was at 1.7cm, dilated and funneling w/ baby A's sac bulging through.  I had no idea, and no contractions other than Braxton Hicks.  Thankfully, I was still in the safe window to have a cerclage and got one put in two days later.  I then was on strict bedrest till 28wks (at home), and modified bedrest after that.  My cervix fluctuated from 2-2.5 for the rest of the pregnancy.  I went into labor at 35w6d.

The main thing to remember is that anything over 2.5cm is great.  Drs start to get concerned once you're below that, and below 2cm will probably get you strict bedrest.  If you get in that range, just try to stay off your feet as much as possible, and make sure your dr checks you often (at least every 2 wks).  Things can change SO fast.

  • I never had any issues with my cervical length and they only monitored it from weeks 12 to 28.  After that they didn't bother looking at it.  Maybe it was because I didn't have any issues??
  • Last measurement at 28w was 5.8cm. They asked me if I had a long labor w my first. (I did). I had them at 37w3d
  • At my 24 week ultrasound I was at 3.3 cm, and it was the same as at my previous ultrasound at 20 weeks.

I was told that my length was totally fine for me, because I'd had a biopsy of my cervix last year (the ultrasound tech wasn't sure how that would affect my length, but apparently it was fine).

  • At 30 weeks (which they said would be my last check) I was over 4cm. My water broke and I went into active labor 2 weeks later.

  • At 23 & 24 weeks it was a little over 4cm. At 26 weeks it had shortened to 2.4 cm and I was put on bed rest. I'm currently 31 weeks. I haven't had a measurement taken since 26 weeks (I will next week) but my OB has been doing internals every other week and she doesn't think anything has changed.

  • At around 17 weeks I was 4.2 cm, 20 weeks I was 3.5, 22 was 2.8 with slight funneling (put on modified bedrest), 24 weeks 2.5, 25w5d 2.0 and went into preterm labor, 2-3 cm dilated with funneling was hospitalized for a couple of days, given steriod shots and drugs to stop labor, sent home with procardia and put on strict bedrest until delivery at 35 weeks exactly.
  • Thank you everyone for your replies! I had a cervix length measurement (via abdominal u/s) last week at 22w3d and it was 2.7cm. I just found this out today at my regularly scheduled OB (actually a perinatologist/MFM) appointment. The nurse and doctor didn't seem concerned, but I know this is borderline at best. I did have a second check yesterday (my doctor sent me for three checks, once per week, after I had one day with pretty intense/persistent discomfort a few weeks ago - nothing like it since) and though I don't know the result yet, the tech said everything looked "fine". She checked abdominally and transvaginally. I expect my doctor is wanting to see if there's any change between checks rather than viewing one result in isolation (assuming that one result isn't dreadfully short). I'm not letting myself freak out and I'm trying to take it easy (I am working but I have a desk job) but it seems a lot of women are put on bedrest for measurements like mine.

Thank you again and any other replies are most welcome!

  • I cant remember exact numbers... but mine was somewhere just above 4. at 28 weeks it dropped to 3 or so... by 32 weeks it was 2.1... and since then the babies have been too low to get a measurement.
  • I was one of the lucky ones.  My cervix always measured between 4.5-5 at every appointment (we checked it from 17 weeks to 32 weeks, every other week).  I will be 37 weeks tomorrow and as of a week ago, showing no signs of anything progressing down there...so my OB thinks I just may make it to my scheduled c-section at 38 weeks.

  • I had cervical checks from 14- 28 weeks and my measurement was never below a 4.5.  Most measurements were above a 5.

  • Mine was always measuring around 4cm.  I delivered at 28 weeks - had nothing to do with shortened cervix or PTL though.
  • My cervix was over 5 until about 28w.  I started to funnel and get soft b/c of braxton hicks contractions, but stayed at a 2.5-3 until delivery day-I had a c-section at 36w1d b/c of pre-e.
