I'm not sure what you mean by a 6" piece of plywood, but we did cut a piece of plywood that was the same dimensions as the mattress to put underneath it so there was a solid surface for the sensor pad to sit on.

6", no? The directions for ours said to use a 16" x 24" piece of 1/4" plywood.
You need to do it or it won't work correctly and you'll get false alarms. I have plywood for the crib and a smaller piece of plexiglass for the bassinet, since they will cut that for free at HD and the plywood doesn't come small enough and isn't available to be cut.

No- our crib is from pottery barn and has a plywood base. We didnt put anything else under there.
