Try some naked time for awhile. My DS also had this a few weeks ago. The only thing that worked is not CD safe. It's called Critic-Aid Clear. We use it in the hospital I work at for diaper rash caused by rough meds like chemo and stuff. It's thin enough that you can just line your dipes with fleece or disposable liners. I did and haven't had an issue. It cleared it up in like 2 days too!

Any idea what caused it? Common culprits are detergent problems, feeding things that disagree with LO, and teething. Where on him is it located? Is there a discrete area with rash, or is it more all over?
I'd try to do as much naked time as possible. We have a waterproof mattress in my son's crib, so I put him in there and accept that I'll be changing the sheet frequently. Personally, I'd probably also go ahead and use disposables and a heavy-duty diaper cream, but I'm quite possibly being influenced by the fact that we had a detergent issue causing a rash a week ago (just finished stripping the diapers yesterday). If there's a large open area or he's bleeding a lot, I'd say it's probably worth a trip to the pedi. He/she might be able to get you a prescription cream.
I'm sorry - I know that's not really a CD-friendly answer. Most of my rash experience comes from work, though, and we'd pretty much always break out the big guns with regard to prescription creams. Also, him progressing from no rash two days ago to bleeding today seems fairly quick to me, so I'd personally want to treat him aggressively.
LO had really bad diaper rash and nothing CD-safe was working. I tried Triple Paste with a liner but it wasn't really working (and I was worried about ruining my dipes). I started using Aveeno diaper rash cream (pinstripesandpolkadots.com says it's the best "3 star" bet) with a liner on Saturday and it's helped TREMENDOUSLY. Still not technically 100% CD-safe, but it's better than some other options out there and I think it will be fine.
ETA - I also made an effort to change diapers more frequently, even if it meant every 90 minutes. Also, I was on antibiotics last week and read that sometimes that can cause diaper rash in EBF babies. As PP said, it's always a good idea to find out what's causing it.
It's not all over, it's where I would expect the most moisture to be. I have no problem using disposables if I have to. I just don't know which kind of cream to get.

And I can't edit from my phone but I haven't figured out what it's from. He's been extra poopy lately so that might be it. we haven't changed anything, but he is only 4 weeks old so everything is new.

DS had it from being poopy pretty much constantly. I think their skin just has to get used to it, to a point. I also figured out that the disposable wipes we were using were contributing to the problem.
