Breastfeeding

drastic drop in percentile?!?!

My baby is 4 months old and has been exclusively breastfed his whole life. I had his four month well baby check up with his pediatrician today and I am a little concerned. He was 50th percentile for weight at birth (8.3lbs). At two months he was 25th percentile (10.14). Today at 4 months he was 10th percentile (13.2lbs). The drop in percentile really concerns me and now I am worried he is not getting enough milk. He has plenty of wet and dirty diapers. He seems really content. I feed him on demand which is generally every 3 or so hours. He eats every 3-4 hours during the night as well. I just wondered if this drop in percentile is normal and if not how I can increase my supply. Thanks.

Re: drastic drop in percentile?!?!

  • That's would scare me too! But what did his Pedi say? Did the Pedi suggest weighing your LO before and after a feeding to see how much he eats? Or Perhaps you can try pumping and giving him milk that way so you can be sure to see how much he is eating. My first step would be the weight before and after eating though, GL!

  •  Is he moving more? many babies weight gain slows when they begin to move more. Increasing supply is basically just nursing more... offer the breast more frequently during the day and nurse in a quiet room allowing baby to finish one side before switching to the other..... for more read these links:

  • My baby is 13 lbs at 4 months and that wasn't 10%. Double check a growth chart.
  • Yeah that doesn't sound like 10%.  I would double check.

  • Something sounds off to me too.

  • I agree with PP - something does sound off about that growth chart.  Try this one.  DD went from 95%ile to 75%ile between 2 and 4 mo, and the pedi was not concerned.  At 2 mo, she was basically just lying down all day, and by 4 mo, she was really active (rolling, bearing weight on her legs with help, sitting assisted), so he said that was normal.

  • My DS had a huge percentile drop between 4 months and a year.   We went from being 95th percentile at 4 months to 10th Percentile at 12 months.  While we were concerned about the drops our first pedi wasn't concerned since we had a very active baby and he was meeting all his milestones.   Our new pedi, whom we started seeing at 12 months, told me that she wanted to see him maintain his growth curve from that point on,  she also told me that babies also tend to fall off their growth curve around 9 months because they become more active.

What does your pedi think?   Is she/he worried about the drop in percentiles?   Is your little one meeting her milestones on time?  If your pedi isn't concerned yet, I wouldn't worry, sometimes babies just hit growth spurts at different times.

  • 50% to 10% when the baby is still gaining isn't actually a problem. He's finding his own curve - how tall is he? If he's a shorter baby, he might have a higher percentile for his height/weight than his age.

Also, I second the numbers sounding off. My 4 month old is 15lbs and he's way higher than 10%. He shouldn't be too far off from your baby percentage-wise if he's only 1.8lbs more than yours.

  • Thanks ladies for all of your insight. I checked my growth chart at home thinking maybe some of you were right about the percentile being off, and he did seem closer to the 25 percent range. However, the link provided below from babycenter put him between 5 and 10 percent, so I don't really know...haha! Anyways, he seems healthy so I am just trying to increase my supply. My pediatrician was a little concerned, but she didn't ask me to supplement, which I was glad about. She does want to see him again in a month to check his weight though. Anyways, thanks for all the advice.
