SDI question - help!

My HR person suddenly went on vacation and won't be back until next Tues.  I need to turn in my disability forms to Kaiser this morning and I have a few questions on the form that no one else at my job will know how to answer so I thought I'd take a shot with you guys since you've been there. 

1) When it asks for date my disability began and last day I worked I write my last scheduled day of work even though I'm still there currently, right?

2) Why did I stop working?  Is 'pregnancy' sufficient or do I need to be more specific about why I am stopping work at 36 weeks?

3) The big one - 'If your employer continued to pay you, indicate type of pay' and it lists sick, vacation, and other.  I have about 6.5 weeks of leave accrued and I have to use my sick time, correct?  My work generally uses sick time to 'pay' employees to cover the waiting period and then I plan to use the rest of it on the other end once SDI and PFL run out before I come back in Jan.  I just don't want my answer to affect SDI or the amount that I receive.

  • Kaiser is pretty helpful will filling it all out, but since I did mine yesterday I'll help you with what I can :-)

    1) Yes, you put the last day that you will be "working"...

    2) "Pregnancy" is exactly what they way to see

    3) I'm not sure... Whether you "have" to use your sick time depends on your company policy. I'm not sure how it will affect the paying of your disability, but hopefully they'll be able to advise you. Have you tried calling the disability office? I found they were actually pretty helpful...

     This is what I put for #1 and #2. 

    For #3, I just put that I was not receiving any additional pay from work.  I used vacation to get me through the week waiting period, but nothing after that.  I figured that SDI wasn't paying me for that week, so it didn't count as additional money that I was receiving.  And if you are using the rest of your accrued time off for AFTER SDI/PFL run out, I don't think they'd care about that either. 

    using my vacation time for the week waiting period and telling SDI that i wasn't receiving any additional funds did not affect the payments from SDI.

    That was my plan as well.  I just got off the phone with EDD and they were really helpful.  The only options were sick, vacation, or other so she said to mark 'other' and then when they process everything they will call my HR dept to confirm.
