Should I be seeing an MFM?

My doctor has never once mentioned the need for an MFM (even though I have a bicornuate uterus, which makes me high-risk in an of itself), but I see a lot of you ladies either had one or are going to one now.  I will say that my doctor has been wonderful so far, and he is fairly blunt and open with answering my questions.

He has me in for ultrasounds every three weeks until twenty weeks, then he said I will see him every 2 weeks thereafter.  However, that is truly all he has done so far...an ultrasound each time, tech does it, takes measurements, blood pressure, weight, asks me how I'm feeling, says things look great, and asks me if I have any questions.  Should he be doing more at this point or am I just being neurotic?  Should I ask him about an MFM?

  • I saw my mfm and my ob.  If I were you I would ask to be referred to an mfm considering your situation.

  • How many weeks are you?  I see you are due in January but that's too much math for me.  Wink  I didn't start seeing one until 18 weeks or so, I would ask him if he has any plans for you to see one in the future and maybe he hasn't mentioned it yet?

    If he doesn't have plans to, I would ask him if he can refer you to one to be on the safe side.

  • I know some people think that having di/di twins isn't enough of a reason to see one, but having your uterus issue is reason enough! Ask for a referral. I didn't see one until 16 weeks, so it's possible it just hasn't come up yet. (I was due in January and at this point four years ago was about 12 weeks along, so I'm assuming that's about where you are now.)

  • IMO, I'd ask for an MFM referral. I ended up only seeing an MFM (mine happened to deliver) and I never regretted having the extra care "just in case." Given your concern with your uterus, I'd talk to your OB and ask him/her to put in a referral.

  • Some OB's are comfortable and experienced with twin pg.  I saw my OB and then started seeing both at 16w when I had my first cervical length check.

    I think if it would make you feel better to have the extra care, ask your OB to recommend an MFM.

  • Sorry ladies, my ticker is down.  I am 15w2d (due January 2).  Thanks for your advice!  My next ultrasound is on Monday, so maybe he will bring it up then.

    Fully agree.  I had not a single complication but my doctor was so on top of things I always felt like she was one step ahead of any complications that could arise.

  • Like pp I started seeing my MFM in addition to my OB at 18 weeks. My OB is very experienced in twin pregnancies, but the MFM has better u/s equipment and the techs are more experienced with multiples & the risks that come with them/what to look for.

    With your uterine issue I would insist on having at least one appointment with a MFM.

  • I've been seeing an MFM but I've only been there twice. The first was around 13 weeks for a genetic consultation and an NT scan. Then I went back about 19 1/2 weeks for an anatomy scan. Those are the only times I've been to my MFM's office. At my OB appointments, they just do the norm, blood pressure, weight, urine and a quick ultrasound to check heartbeats and all that. My last appointment was last week when I was 20 weeks and they did nothing different except measure my belly.

  • I wasn't sure if I'd been sent to one or not based on my first couple of visits with my OB.  Then one day she came in and said "ok and from now on you'll have your sonos done by the MFM" that was right at 20 weeks.  So just ask WHEN (assume you will be) you will start seeing the specialist.

  • I saw my OB for everything for the first 20 weeks, and then started going to see MFM for all u/s at that point and seeing my OB for regular appts.

  • Thanks, ladies.  I'll definitely be asking at my next appointment, and I'll take your advice and ask "when," not "if."  I was worried about hurting my OB's feelings, but that's not really something I should worry about.

  • I never saw an MFM or Peri or anything. I felt that my OB was being proactive enough...even after going into PTL at 31 weeks they helped me stay pregnant until 37.5!

    However the twins were the only high risk part of my pregnancy...I didn't have any other uterus or cervical issues or anything.

  • I saw a MFM once around 13 weeks to do a scan on Hannah's (at the time, Baby A's) nuchal fold since at my fertility clinic, something seemed "off."

    After that I was under the care of my OB only.  I asked about a MFM, but since my only high-risk factor was the twins, I stuck with him only.  He said if I had anything else pop up like GD or blood pressure issues, he'd send me right back to the MFM.  Thankfully, I had a very uneventful twin pregnancy and didn't go back.

    If I had your uterus, I'd see an MFM for peace of mind if nothing else. 

  • I see my ob once a month mainly to ck my weight, measure and see if I have questions. Every other week I see a peri or mfm for a level 2 u/s to monitor ttts and measure twins ect, it was set up by my ob. I'll be doing this thru 2nd tri, I forget what they said after but I think since the risk of ttts is a 2nd tri thing they may not see me every other week once I start 3rd tri. I like seeing them, they seem to really explore whats goign on with the twins and are very informative! my ob just listens for heartbeats monthly. If you are concerned I'd ask to have them recomend one for you, couldnt hurt.  GL
