Well technically I suppose the second twin would be the middle child, but not in the traditional sense. My cousin has a daughter and then twins and I don't think they consider one to be the middle.

I am the oldest in my family and have 2 sisters that are twins. We always call the first twin the "middle." She was born a whole minute before the other one
