Kaiser - STD requires doctor's note questions?

I hate how confused I still am by all this stuff.

I talked to my doctor today in my 32 week appt and asked about getting a note to go out (and on SDI) at 36 weeks.  She said I just bring in the EDD form to the medical secretary when I'm actually off.

But my work form "Request for Short Term Disability" states that I need a note with the date of my disability (I assume this is the 36 week date, right?), percent disability, blah blah blah.  I need to submit this form and start my "leave of absence" paperwork with my HR soon (supposed to be within 30 days, which is fast approaching), but Kaiser wants me to wait until the date of my leave?  And how do I get the leave without the note?  I'm so so confused.  Is there any reason Kaiser wouldn't tell them I'm disabled as of that 36 week date?  Is that standard?

Help please!  Will the medical secretary give me a note?  I have their number to call, but of course I dialed them at 12:33 and they go on lunch 12:30-1:30...argh!

  • I don't have Kaiser, so I can't help with that.

    But my work also required something in writing from my doctor (with 30 days notice as well) stating that I should be on STD as of X date. He just wrote something really brief on office letterhead and signed it.

    I would think this isn't the first time your doc's office would have had a request like this from an employer, and that someone there should be able to give you a note or will fill out the form for you if you bring it in.

    GL! I know how confusing it all is. Really frustrating!

  • Take everything to the medical secretaries, they will have your doctor sign off on your paperwork (and have you fill out any other necessary forms).  You won't actually have your doctor do anything, the medical secretaries take care of everything for you.
  • Your doctor can very easily pull up your info while you're in his/her office, check a little box that says you're disabled and cannot work for x amount of time as of x date, hit print and give you the copy. I've been written out twice for pregnancy and that's what my doctor did. If your doc can't be bothered, go the medical secretaries and they will get your doctor to sign off and handle that part. You can't file for SDI until you are off work. But your private short term disability is a different story, and I'm sure your employer wants the doctor's note for their records as well (mine did as part of my FMLA paperwork). So someone needs to get it for you, although they can wait until the date you are written out, they don't need to give it to you before. Would be nice if they did, but they don't have to. The medical secretaries will help with your private insurance as well - but keep in mind, most private short term disability insurers will not pay you for the 4 weeks prior that California deems you disabled. Just because your doctor wrote you out, it doesn't make a difference to them.

  • Every pg I've had with Kaiser printed out something for me saying I'd start disability on x date (4 weeks before due date) and can return on x date (6 weeks after).  I got mine at my 30 week appointment this time around.  Two copies actually, one for work and one to turn in to medical secretary to attach to EDD form.
  • Thanks guys, I think I've got it now.  My HR is just completely unhelpful and I kept asking her what I needed and she would say this doctor's note associated with the STD form.  Turns out I don't need the STD form.  I (think that what I) DO need the standard "doctor's note" form (like what tri-reader is talking about having gotten) from medical secretaries stating when I can go out and return, etc, and not the note saying I have some reason to be out before that.

    I just wish my HR could explain these things to me.  Good HR people are worth their weight in gooooold, I swear.  It is IMPOSSIBLE to get any answers at my workplace.

    Ditto this.  At my OB visit I just had my doctor print out a paper stating when I was going out on disability.  He printed it out and handed it to me to take to my employer and to the medical secretary.
