We got DS a kiddimoto balance bike: the orange superbike scrambler. DS loves it; it looks like a motorcycle.
I have also heard good things about the scuut bike at REI. That was our second choice.
For a helmet, I would recommend going to a bike store and finding one that fits their head properly. I don't remember what brand DS's is, but it was hard to find one that fit his skinny head.

We have a strider bike and I'm not sure the brand of helmet. I had to go to a local bike shop for the helmet bc the ones at target, walmart, TRU were all too big. But she has a tiny head for her age.
We considered Skuut (ran across poor reviews re: quality) and Like a Bike ($$$!)because that's what you typically see in our neighborhood, but ultimately went with a Strider and it's great. (They're based in my hometown, so what was weirder was that when my mom called to place the order so that we could get it shipped to us overseas, it one of my high school skiing buddies on the other end of the line!)
He absolutely loves it and could ride it from day 1! We got it because although he isn't short (50% in height), he couldn't reach the ground on any other bike we tried him on in the stores (even with the seat lowered). The 10" vs 12" wheels that this bike comes with made it possible for him to ride it now and not have to wait 6 mos - 1 year to be tall enough to ride it. We bought the Cars helmet that was suggested also. However, it is far too large so we'll have to purchase another helmet.
