Success after IF

ICSI question

i feel kind of silly about asking this since this was done in 2008 and I havent wondered about it until now..

when we were TTC#1, we did ivf and they did ICSI. i know what ICSI does, but not why they do it..it was never explained to me as far as i can remember..

so what is the reason behind it? mh went to SA and from what i remember everything was normal. i also remember doing a cervical mucus test and they didnt find any moving sperm in the sample. everything from that time is such a blur, i cant remember what that means.

we are ttc#2 on our own for the time being w/ just charting, because for the first time in my life i am having close to normal cycles (36-39 days instead of 90something days lol) and i just was wondering if PCOS wasnt our only issue, and that maybe mh had an issue as well.

thoughts? thanks!

Re: ICSI question

  • We did ICSI because we had really bad MFI -- lowish count, but really bad motility and absolutely terrible morph., which my RE believes effects the ability of sperm to fetilize eggs.

It can also be done just to improve some outcomes and fertilize more eggs, because they hand-select the best sperm.

  • I had ICSI done because of my husband's low motility as well as a somewhat low count.    I am not sure why they would do it if there is not a male factor problem.  Now I'm curious too!:)    Best of luck  with #2!!!!

  • They do ICSI if the sperm can not get to the egg or if it can't penetrate the egg and fertilize on its own.  My DH's SA came back within the normal range too, but too close to borderline for my RE so we did 1/2 ICSI to see if that seemed to make a difference.  It didn't and this time around we didn't do ICSI at all.

  • we did ICSI because we had 0-1% morph.  We did get pregnant 2x on our own (well with clomid and ti) and both of those ended in losses so we weren't sure if that had to do with the sperm quality and didn't want to risk more losses as you can only be on clomid so long for each time.
1 chemical and 1 loss at 9 weeks prior to DS

We also did ICSI for MFI issues.  My husband's SA numbers were great for everything except morphology (<2) and ICSI helps with that since it aids fertilization and low morph can result in low fert.  As a result of ICSI (we did it with all 4 of our IVF cycles), we had very high fertilization rates (low 90%s every cycle) which was helpful since I also had a lower egg quantity from all my cycles so it helped us maximize our chances of getting an embryo.

Personally, I think it's worth it to get another SA again if you don't have luck TTC#2 after a few cycles b/c it can change over time and you would want to know if this is part of your Dx now (although I hope not).

  • They may also do ICSI if you don't have very many eggs that are mature at retrieval...just to be sure that you get something to fertilize. We had 10 eggs and we did ICSI on half and the other half natural fertilization. I really felt that there was some benefit to natural fertilization because I felt like the strongest best sperm would make their way into the egg...and they can only tell sperm quality from appearance and I think there is more to it than that. So what looks like the best under a microscope might not actually be the best if you know what I mean. Also I didn't want to naturally fertilize all of them on the chance that we actually had a fertilization problem in which case we'd end up having nothing to transfer. So that was why we did 1/2 and 1/2. And actually the two we transferred were natural fertilization...I don't think anything fertilized with ICSI.
  • thanks for the info guys! if we arent successful on our own w/ just charting by the fall we will be going back for FET, we are lucky that we have 17 more frozen.

the whole getting pregnangt thing is just so tricky.  We did IVF with ICSI because my husband had a low count/motility.  We had tried for 3 years.  4 months after I had DD I was pregnant again.  Imagine my shock.
