9 cm dilated but at a -1 station. What does this mean?

My SIL is in labor right now, she has had a long labor so far. We just got word that she is now at 9 cm dilated but her station is still the same at -1. I know that she still has 0, 1, 2, and 3. I understand the stations and where the baby's head is. This means she hasn't made the decent into the birth canal. 

My question is that with her being as dilated as she is but the baby still not having dropped like she needs to, does this mean she'll have to have a c section or can she still deliver?

Sorry, a total newb here. It confuses me with her being as dilated as she is but being stationed so high up still.

    I'm not entirely sure but could be because baby is in a bad position (which often times can lead to a c-section or very long pushing time) so is she able to get up out of bed (assuming she's there in the first place) and move around? They should be able to monitor even if she's up and about. I'd advise her to move around as much as she can to get baby in a good spot. And to push in an upright, gravity friendly position. The worst thing in the case that baby is big or in a bad position would be to lie in bed the whole time and push on her back. Good luck to her :)

    If nothing else, she can turn from side to side with one leg up, one leg scrunched, in motion, to corkscrew the baby down. But if she's at all able to get up and move she needs to do it, now.

  • I don't think it means anything. Some people have long labors. She could just be going through a natural alignment plateau. Like PP said, getting up and moving is best for moving the baby down. Walking and squatting are key, because the open the pelvis. Laying on her back at this point is very counterproductive.

    Those numbers don't look that great.

    The baby probably isn't descending due to bad positioning, the cord being wrapped possibly and keeping the baby from dropping down..... I'd guess c-section with those numbers.

  • When I got to the hospital with my son I was 8cm dilated and he was very high up.  I labored a long time before he eventually descended but a c-section was in my future if I didn't deliver when I did!

  • I had a hard time getting my first DD past 0 station when I delivered her. I pushed in a squatting position which got her down from -1 to 0, but they used the vacuum to help deliver her.

    This was the situation with my first.  Unfortunately the cord was so tight around her neck that it was preventing her from getting to the birth canal.  I couldn't push her down because when I tried to push the cord would tighten up and she would go into distress and they could not reach her with the vacuum.  It ended up in a C-section.  The c-section wasn't too bad though.  Sometimes it really is the only option.  I am sure both mom and baby will be healthy, but the only people who really know anything are her doctors and nurses.  We are all just making guesses.  Good luck!!
  • I had a similar situation when I was in labor with DS.  I was only in labor for 2 hours when I got to the hospital, and I was dilated to 6, but DS was still very high.  Within another hour, I was dilated to 7-8, but DS was still very high.  From hours 3-8, I was stalled at 8.  I don't know why the doctor was so nonchalant about it, but at hour 8, I was finally given pitocin.  I was still contracting well, but I was no longer dilating. 

    I wasn't checked again until hour 10, at which time I was 10cm, but DS was still high.  When I was checked again, we found out why I stalled at 8, and why he was so high still.  He was face up, which makes it much harder to move down.  In addition to that, he was almost 9 lbs, which makes it even harder to move him down when he is face up.  They were able to manually rotate him, and he moved down pretty quickly, and I delivered within 30 minutes.  Other than them rotating him, I didn't need any assistance such as the vacuum or forceps.

  • My SIL went through something similar (not sure of the exact station numbers) with her second.  She was fully dilated by baby hadn't dropped.  She squatted using a bar for about 15 min. and the baby dropped almost instantly and was out very quickly after that.
