SA significantly worse than 1st round...opinions

First off, I know this could be much worse but I am over analyzing this cycle and am so anxious for some reason...His count is fine 40mil though that is "significantly" lower than last time we went through this...not worried since we're doing IVF. His morph is 3.5% which nurse played down but after googling seems like best practice is ICSI...thoughts? Insurance won't cover ICSI at this point, we'd have to have a failed cycle.

Also, we totally forgot and had sex the night before...how much does that impact Morph (I am sure it reduced the count). Is it worth another SA in a couple weeks? I don't want the nurse to think I'm nuts asking for these things if I am totally overreacting.

ETA: spelling :/

Re: SA significantly worse than 1st round...opinions

  • First off, if you have questions, try not to hesitate to ask your nurse or doctor.  I've asked plenty of questions of my RE and his staff - that's their job!  :)  Hopefully you feel comfortable going to them with any questions or concerns.

My husband's Strict Kruger morphology was 1% when we did IVF and they did not do ICSI.  I don't know if you had a Strict Kruger or WHO though.  According to Kruger, 4% and above is normal so 3.5% seems good with that criteria.

As far as having another SA, they always told my husband to abstain 3-4 days prior to the test.

    Good luck!

I know it's their job, I just asked so many today so I thought I'd get some feedback before looking like a lunatic. When she said they looked for 4 so 3.5 wasn't too bad, I felt satisfied until googling. Knowing you didn't do ICSI makes me feel better...stupid Dr google!

And, I can't believe he didn't abstain...total lapse in judgement on our part obviously.

  • Well.... our SA is what sent us to IVF in the first place.

DH's best one was 37 million/ml, with 28% motility and 1% morph. The first one was something like 17 million/ml, 1.5% motility and 1.8% morph. It was so bad we couldn't get a post-wash count!

How much more does ICSI cost for you? I didn't think it added a whole lot to the price generally.

We did ICSI, but it's generally standard practice with my RE and was included in our overall cycle price.

Sex wouldn't impact morph, IMO. Count, yes.

ICSI is $2500, in the grand sceme of things not that expensive but insurance fully covers IVF so DH isn't really wanting to spend $on ICSI since if it fails, we would be able to get it covered. I would rather go through this less times than wait and see what happens. I am going to call and see if they will do another SA after he abstains to see if it helps.
