I have heard that once you heat up BM, it has to be used within an hour. I don't know about the other questions... I would guess that it would depend on the effectiveness of the insulated bag, but I really have no clue.

Once heated a bottle of BM should be used right away. But may be given at the next feeding (if you put it back in the fridge between feedings). Freshly expressed breast milk (never in fridge or freezer) can technically sit out for HOURS without spoiling, though I don't encourage people to test that theory For the insulated bags - depends on the brand. I believe the Medela bags state 8 hours.
