I have a rebate form for $15 with the register receipts for the purchase of 2 tubs or boxes of Enfamil Premium Newborn or Infant Powder (23.4 oz or lrgr) or 8 cans of Enfamil Premium Infant Ready to Use liquid (32 fl oz). It's only one per household and no copies). It's good until the end of the year.
