VBAC

lurker w/ a question re: pitocin

One of the midwives in my practice said that if I don't progress while trying for a VBAC they may give me a "tiny bit of pitocin" to help get things going.  All I've read said that pitocin is a big no-no while going for a VBAC. 

If you've had a VBAC, did you have pitocin?  Will you decline it if it is offered?  Is there some research/data that I haven't found yet that shows pitocin isn't going to cause rupture/the data shows it is such a small risk?

  • I'm sorry I don't have any links to the research but I do know that low doses of pitocin are considered safe. It's a MUCH lower dose than would be used for a typical induction.  I also know that some OBs refuse to use any pitocin at all and would instead have you go right in for surgery if it were needed.  I am comfortable with a low dose and wouldn't mind it being used to help my labor along.  I'm not sure how I would feel about being induced with it but I'll cross that bridge when it comes to it.

    I would discuss with the MW how it would be used and how much and and something I'm personally interested in is how often they've found it helpful in such a low dose, something I just realized I wish I had asked at my OB consult!

  • There are some studies showing that labor induction/augmentation can be done safely in VBAC moms as long as a low dose of Pit is used.  Here are a couple I found during a quick search.

    I'd feel comfortable with a little bit of Pit, especially if my only other option were a c/s.

  • I got pitocin after I had stalled for a bit. I'd already been in labor for a full day and never thought to decline something to help me progress. ACOG says it is not contraindicated, and though the risk does increase, it is still very low.
  • I was given a "whiff" of Pitocin when I was stuck at 9cm.  I held off on getting Pitocin for as long as I could, but eventually, it felt like the only option.  Shortly after, I went to 10cm and had my VBAC.

    In all my research, the only drug that I found that has been proven to be unsafe for VBAC is Cytotec.  Though Pitocin is not recommended, I have not found any research that proves that it is harmful.
