I received my calendar for IVF#2's protocal today.
Holy Cow! My how things have changed. Even thought I changed facilities (not RE's) this is completely different and left me with 8 million questions!
So here's the change:
I'm on 20 units of Lupron to begin, not 10. They want me to take Folistim, not Gonal-F. They want me to take a baby asprin daily. They have me taking Medrol from the day of ER until ET. The want me on 1ML of PIO (which I was last time) but they also want me on 200mg trans vag tables of Prometrium 2x a day (dude seriously), and the kicker: 2 Ovidrel Trigger injections. Plus they are changing the culture our embies are kept/grown in.
Sheesh! It's so different! They've added so much more.
I did e-mail IVF coordinator to ask her about 12 different questions, and I felt like she gave me a reply that came across as agitated with my questions.
I feel like I have every right to question the cycle. I'm not questing their reasoning, I just want things explained to me. Like why I'm on Prometrium, why 2 Ovidrel injections, and why Medrol. I'm not someone who just does what I'm told, if I don't understand, I ask! So get snotty all you want!
Anyways: ER is tenative for 5/4. Then we have to decide between a 3dt and 5dt (if we have enough). This clinic prefers 3dt, because they believe it's better for the embies.
5/4 seems so far away, but will be here quicker than I can imagine!

gonal f and follistim are the same thing, and they can be used interchangably. If u liked follistim, u can def ask to be switched.
The steriods can help with implantion, and both my clincs have used them.
The baby asprin seems to be a clinic to clinic thing. Since it is safe...some RE's toss it on. My current RE took me off of it.

Also, my new RE is partial to 3dt too. Also since ur last cycle was like mine...here is what he told me. "You were doing great on day 3, and not great on day 5...so i don't see why we would repeat that, lets get them back inside while they are still looking good".
i've always done medrol, follistim and 2 ovidrel for trigger. i do crinone instead of pio, but it sounds like they are doing a sup/inject combo.
But i can't believe the nurse wasn't more patient with you. i mean isn't that her job to make sure you feel comfortable and informed?

Here are some of my thoughts on your changes:
I have no idea about the lupron dosage.
Follistim and gonal-f are the same drug manufactured by two different companies. My RE lets me use either or both on any cycle.
Many REs have IVF patients take baby aspirin during their cycle. Some say it helps with uterine lining and a few other things. Others don't see the benefit unless you have a clotting issue. I have clotting issues, so my RE has me take it.
My RE has all of his IVF patients use PIO and endometrin supps. While there are risks from not having enough progesterone support during an IVF cycle, apparently there isn't a problem with getting too much. It is my understanding that PIO goes through your blood and shows up in b/w while the supps go straight to your ute and do not show up in b/w.
My RE uses one ovidrel for trigger. But one ovidrel injection has less actual HCG than some of the other triggers like novarel. Consequently, most of the other doctors at SIRM either use the other type of HCG that has 10,000 USP units or they use two ovidrel injections (I think the conversion has ovidrel at around 7,500 USP units).

Dx: DOR, Endo, APA+ (really high beta 2 glycoprotein antibody and high everything else tested), heterozygous MTHFR mutation, positive for lupus anticoagulant, high FSH, low AMH and both tubes blocked (per HSG on 3/8/11)
IVF #1 - long lupron (with HGH, intralipids, lovenox and BA); 4 retrieved, 3 fertilized; ET 2 blasts and 1 frozen = BFN
IVF #2 - a version of antagonist with EPP (with HGH, intralipids, lovenox and BA); 6 retrieved, 4 mature, 3 fertilized, 2 blasts and 1 frozen blast transferred on day 5 = BFN.
IVF #3 April was postponed to May, May was canceled. June/July was canceled. Had a cyst aspiration and then began IVF #3 in August. ER on 8/22; ET on 8/24 with AH. +HPT on 9/5. Beta #1 (11dpo) = 3; Beta #2 (15dpo) = 29; Beta #3 (17dpo) = 60; Beta #4 (19 dpo) = 118. Heartbeat at 6 weeks 6 days =132. Lil is here!
TTC#2: Trigger + TI = BFN; Clomid + Trigger + IUI = BFN.
IVF #4: BCP + MDLF + Lovenox = 7R, 1F = Transferred 1 6-cell embryo on day 3 = BFN
IVF #5: MDLF + Lovenox = 4R, 1F = Transferred 1 10-cell compacting embryo on day 3 = BFN
IVF #6: (New RE): Long Antagonist November 2014 (transferred two 8 cell grade 1 embryos and froze one blast) = BFN
FET#1: BFN

Follistim=Gonal-f, the delivery system of either have pros/cons but they're essentially the same.
Medrol (steroid) may help with implantation.
Aspirin may help, won't not hurt, some docs throw it in.
You can't take too much progesterone.
I believe, 2 Ovidrel=1Novarel and is what my dose is for IVF trigger. I used one Ovidrel with my IUIs, I suppose the extra drug is for the extra follicles.
