Projectile vomiting once a day- normal?

DS has acid reflux and is on meds but we also suspect a dairy intolerance. I am on an elimination diet which seems to be helping quite a bit. However, he has had bouts of bad projectile vomiting. At one point it was after every feed for 24 hours. Pedi suspected a virus and had us give him pedialyte only for 12 hours and slowly wean back to nursing. Since then he has projectile vomited on average one to two times a day. It is usually directly after a feed. Sometimes it happens right when I sit him up to burp him, sometimes when he is still laying down eating. It is definitely not just normal spit up (he does a lot of that too). It is forceful and shoots out a good foot or so. He seems hungry afterwards. Normal?

  • I don't have an answer for you, but I was wondering how you determined it was vomit rather than spit up.  My daughter projectile spit up last night, but I was wondering if it was really vomit. It was different from normal spit up in that it forcefully flew out of her mouth .I plan on asking at the pedi next week, but was wondering how you would describe the difference between vomit/spit up. 

    How long have you been on the elimination diet before you noticed the difference?

  • My doctor told me that the difference  is the force at which it is expelled.  Spit up generally does not shoot out forcefully and  does not go far.  If it is projectile it is most likely vomit.

  • jumping in on this one too. Ds has gone back to spitting up a bunch lately again. We started zantac 3 weeks ago and he seemed better for a while.

    Last night he was laying down flat as I was changing his diaper about 20 minutes after a feeding and it came up out of his mouth like a fountain maybe several inches and then went off the side of his face.  Had he been sitting up I'm not sure how far it would have gone. you always here the " if it goes 2 feet across the room thing...".

    But My ds is always over my shoulder head kind of down or laying down when he spits up. But it's forceful and you hear it hit the floor.

    This post has me rethinking now, maybe it's vomit? It's very clear though like milk though.

    I wonder if he zantac isn't working anymore...

  • we r also using zantac and her spit up has become less. our pedi told us if it shoots across the room its likely the bottom stomach muscle that is too tight and requires surgery to be fixed. is the baby still making dirty diapers? he said as long as she poops its just the acid reflux. we also put rice cereal in her bottles.
  • We have DS on Zantac, too.  The meds do not eliminate the spit up - DS does his fair share of spitting up - they just lessen the amount of acid in their tummies so that they're not in pain.  We don't have the projectile vomiting, but he does have bouts sometimes that are HUGE amounts of spit up compared to the usual little ones he has.

     This. When he spits up he generally just turns his head to the side and some dribbles out- probably a few tablespoons or so and he doesn't retch at all if that makes sense. When he vomits he retches a bit and it usually comes out in 2-3 spurts (gross I know). It comes out forcefully and shoots out rather than just dribbles. It literally is the whole feed- several ounces. I have purposely spill 2-3 oz of fluid on the counter to make sure I am not over exaggerating and it is definitely that much. Hmmm. I know that Zantac has nothing to do with the amount of spit up- it is just supposed to neutralize the acid so my worry isn't really that the meds aren't working...I am just wondering if the projectile vomiting every day is consistent with "normal" reflux symtpoms.

  • We kinda went through this with our LO. Mine has bad reflux too & threw up almost daily for a week about 2 weeks ago. He had done it maybe 2 or 3 times before that. It turned out that he was just eating way too much. I let him comfort nurse whenever he wanted, so he'd sit on the boob for so long that his tummy would be too full & he'd spew. Like 4-5 oz a few times. It was miserable for everyone, & steam cleaning the couch/chair/mattress/rug daily was no freaking fun.

    It wasn't an issue when he was EP bottle feeding because he'd get 3-4oz & be done. Then take his paci. Now he refuses to take a bottle anymore or paci (from anyone in any situation- we've tried) so its hard to know exactly how much he's gotten & hard to calm him without a boob. Plus I have an oversupply issue. What we've done is I'll pump a little before feeding then only give him one boob. I'll do "block feeding" I think is what its called where you only use one breast for a 3-4 hr block of time. So if he wants to nurse again 2 hrs after he just ate, he goes on the same side.

    We also hold him upright for about an hour after each feed (which SUCKS esp at night, but its gotta be done) & don't jostle him around much after feeding. Just burp & hold. He usually falls asleep which helps him digest it all. Diapers are changed before feeds & not sooner than an hour after.... unless its really bugging him, then 30 min or so or we'll be super quick & change him on an incline so he's never flat.

  • Oh & absolutely no car seat within an hour or two of feedings. Those always end in disaster because of the added pressure on his tummy the way he sits in it.

    & when I said "hold him upright" I mean chest to chest/tummy to tummy with legs limp, not folded up. It helps him stretch out & keep the food down since his tummy isn't squished.

    Good luck! I remember you were about to try the colic calm when I was. Did it work for you? LO hated it & got black projectile vomit on our lazyboy the 1st time (within a minute of giving it to him). The 2nd time it was a battle of wills to get it in him, then nothing different- just black poop that I'm pretty sure won't come out of the cloth diapers. No less burping, gas, spit up, hiccups....

    If its any consolation, LO seems to already be starting to grow out of it. I didn't have it in me to hold him for the full time the other night, so I inclined his co-sleeper on a pillow & put him in bed between DH & I. Usually he'd start spitting up or even puke before, but he slept like a champ. I have sometimes cut that hour down to 45 min now :) Just keep thinking that this too shall pass!

    Pyloric stenosis. My LO spit after every feed, usually forcefully and they sent him for an upper GI to rule this out. It turned out to be reflux. It is supposedly more common in males than females

    We are using the colic calm and it does work. Yeah the black spit up and icky poop isn't fun but he's definetly better.  Still fussy and the increased spit up has just started back again. blah. 

    Last night was his first really rough night with gas in over a week, so i wonder if it's not working well anymore.,.? hmm.  Argh! It's so hard to figure all this stuff out!  I'm sorry it didn't work for you! It's not a super miracle stuff, but it definetly has cut down on his symptoms.
