If they've got a big ol' poo that's hard and won't come out no amount of water, juice, prunes etc. will help... the longer it stays the worse it will get b/c the colon keeps absorbing water from it.
So, either an enema or a glycerin suppository. They make both for little ones. It's not pleasant to administer but used only occasionally are extremely safe.
We load up on miralax (he already takes it daily but I up his dose if he seems to be more constipated than normal) and if that doesnt work I add prune juice to all his bottles and do a suppository at bed time-- I've found they stay in easier and he wakes up feeling better with a full diaper.

While the other suggestions probably work better, the other thing that helped us was taking a rectal temp.
