DH and I are going on vacation without DD, who will be staying with my parents for a week. Has anyone out there completed a "permission to treat" type of form in case they need to take her to the doctor? How do we make sure she will covered by our insurance and the doctor will take her? She will be going to my parents' so will not be near her normal pedi. TIA!

Search your insurance website and find a local pedi and a local urgent care/ER. I have written notes saying that while out of town (insert name) has permission to make medical decisions on my behalf and that they are allow to obtain medical info as well. I bet you could find a generic Release of Information (ROI) for a mutual information exchange on the internet. When we went out of the country we actually had one signed and notarized just in case. I'd also write up directions on where to go and how to get there and your pedi nurse line (they will be able to advise in a non-urgent situation).
I put all of that in a pouch labeled medical with info about allergies and medications (my youngest has a rare condition and allergies so it pertains) and their insurance cards and your contact information. Sometimes Dr. office will treat but will consent over the phone. In all the times we have left the kids, we have never had to use it but it is good to be prepared just in case.

Lucikly, my parents live in a small town where everyone knnows everyone so getting in to a doctor won't be a problem (they still go to the same Dr. I went to growing up). I am searching my insurance website and already had a copy of insurance cards, etc. for them. I just didn't know what others had done in the past.

Most peds offices will have a form that is just a standard form - I know Woodinville Peds does on their website.
