OK, so here's the background: DD was born at 33w6d due to pPROM. My water broke and when we got to the hospital, they did a biophysical profile and she scored a 0/8. We also found out that she was IUGR (not too badly, though, she was 3lb12oz at birth). She was healthy when she was born, and had a 16 day NICU stay due to jaundice and eating issues. She never needed breathing help. They sent out the placenta and couldn't find any issues. I was running a slight fever upon admission, but they didn't find infection in the placenta. The ultrasound tech who did the biophysical profile was brand new and didn't seem to know what she was doing, so I'm not sure I 100% trust that 0 score, but in the words of my midwife "A 0's still a 0."
So - what kind of monitoring would you want in pregnancy number 2 given this info? I'm kind of seeing this as two different issues - first is the obvious, the pPROM. I still have to do some research on the 17P shots as they relate to pPROM vs premature labor, but I have a feeling the midwives will say they aren't necessary. The 2nd issue is the IUGR and the terrible biophysical profile. We still have no clue why that happened - who's to say it won't happen again? And what if this time my water doesn't break? What then? I definitely want more ultrasounds (with DD, the last one I had was at 20 weeks, and was the anatomy scan - all was well then), but should I also be asking for NSTs?

DD was an unexplained PTL (PROM'ed just shy of 35 weeks). I was terrified with this pregnancy- I didn't feel like I could just accept an "unexplained" reasoning.
This time around I went to a different OB. She specializes in high-risk pregnancies and even though I'm not labeled HR this time around she took me in. Find a good OB who understands your concerns and has experience with complicated pregnancies. Chances are you'll go to term if there isn't a physical issue with you personally, but a good doctor is going to go above and beyond to help out a mom who has gone through something traumatic before.
This time around my OB is very sensitive to my situation. I had 5 u/s before 13 weeks, and 3 more since. I started weekly appointments at 30 weeks. She did my Group B strep test earlier this time (I'll do one next week as well) and did an fFN check last week (two weeks before DD's delivery date) to help put my mind at ease. She sent me for an NST without a second thought when I was freaking out last week and said to come in any time that I need the reassurance. She also checks in with me emotionally at each appointment and takes the time to review all my symptoms as we get closer and closer to when DD was born.
Take the time to find the right OB- someone who will listen and not make you feel like you're over-reacting to every twitch and tingle. And take heart knowing that lots and lots of preemie mammas go on to have full term babies- this board is full of success stories.
I was on the 17p shots when I got PPROM at 28 weeks. However, I believe it was the progesterone shots that may have kept me from getting contractions and going into labor. My water was able to reseal itself after 8 days, I don't know if the progesterone shots had anything to do with helping that. However, I do know that my OB and most OB's want you to go to a MFM specialist after any pre-term delivery and the specialist will almost always put you on the 17p shots to try and prevent pre-term labor again. I was put on the 17p shots due to my DS being born at 32 weeks. I had a full term delivery after my preemie, but they still thought it best because of my past to start the shots with this one, which I am glad they did. Tell your OB about all of your concerns, I'm 95% sure she will refer you to a specialist especially because of the PPROM and IUGR you experienced in the past.

I had Andrew at 26w2d due to IC. We saw an MFM prior to TTC #2. He recommend a cerclage (we opted for abdominal) and said P17 shots (16-36 weeks) are very effective.
I would see an MFM. You need to find someone you trust. And do your research before you TTC. I was really glad we did because I ended up getting my cerclage placed prior to pregnancy with larapscopic surgery and avoided a c-section that way.
