  MIL gave her one from Longaberger (DH is from around where their headquarters is in Ohio).  This is their basket this year.  They are great quality and we'll definitely use DD's forever.

  I bought the liner at PBK last year.  I really drug my feet and ended up having to get it at the last minute, but it was a one-time purchase so I was fine with that.

  I actually bought the basket at Michael's for much less than their baskets.  Oh, and I got the small liner and it is much bigger than I thought.
