So my Mom reminded me last night (!!) that I should prepare a Child Care Authorization form that authorizes her to make any necessary medical decisions in case she has to take Z to the Dr, etc, during our trip.
In my research I found it should be notarized. Big problem, e.v.e.r.y s.i.n.g.l.e notary in a three county area is either unavailable or closes way before DH gets out of work.
Has anyone heard otherwise, or know otherwise. I'm thinking of just leaving the signed form with her and taking a blank with us in the event of an emergency and needing a notary where we are and faxing if necessary.

While we never had to use it, both of the pedi's we have had (kaiser & non-kaiser) have had forms for us to fill out. My mom also has a signed copy of a letter giving her permission to treat. If DH and I were really out of town and the letter didn't work I would hope my sister would just pretend to be me. She looks enough like me that it might work.
Lol, A. We are nearby anyway, in Napa. We can be here in 3 hours. You just never know. You have given me a good idea to file one with our Pedi's office. For now, she is the OOT contact on all our medical stuff anyway.
