Does anyone go to this group? Do you know if they charge you if you need a form filled out? A friend told me her doctors office charged her $10 for every form she needed filled out during her pregnancy.

    I'm not sure if it matters what type of form it is, but I just had them fill out my FMLA forms and it was $25 total for all of them.

    They have a one time fee for FMLA forms of $25.  If you and DH both need them filled out, it's still only $25 total.
