Honestly I had tons of snacks and new toys but he still wanted to wander around and was fussy. Our savior was the portable DVD player with baby Einstein!
For 4 hours, I wouldn't overpack too much. DS really liked the mega blocks we brought in our carry on. We also packed a few books. But our son is like the pp's. He just doesn't like sitting still, even with toys. He wants to get up and move around. I usually end up "taking walks" with him quite a few times to keep him happy.

If you want to kill some time, wrap the new toys like gifts and let them open one every hour or so. DD loves opening presents, and this helps to occupy her for a bit longer. Try to stick to the schedule ususally followed- if it is lunch time, feed them, if it's nap time, aim for a nap.
We haven't had any problems and we've flown with DD 8 times since she was little! On our last flight in December, DD just liked taking laps up and down the aisles and looked at people. haha.
Portable DVD player if there's no in-flight entertainment. A portable magnadoodle is great also! A new book is good for DD as well. Snacks- especially something that takes a while. Also a pain killer in case their ears bother them with the pressure change.
You have it covered. I would just bring a bag of a bunch of small toys and as LO gets bored with them, give LO another toy and keep switching it out. Books will only last so long.
We flew last weekend. She fell asleep on me for both legs of our returning flight and I actually realized that I could have read something. So don't forget about yourself. If there is a snack that she LOVES then you should take that one. My LO loves yogurt melts and snacked on those the whole way up and down. The sucking helped her ears. I also had just a shirt in the diaper bag for me. Other than that your list was my list. I kept it simple and she did great. We were also on the plane for about 4 hours.
Wet wipes - wipe everything down on the seat in front of you and the arm rests.
Any smaller toys that will keep her occupied and aren't noisey.
With a flight that long I would do a few gallon bags - each with different things to play with and snacks. This way, after the first hour, you can switch it up.
Life is either a daring adventure or it is nothing at all.

We are flying 3-4 hours next week and I plan to bring the following:
1). A couple of (quiet) new toys (crayons, books, blocks)
2). A baby movie or two (we got her baby headphones)
3). spare bottles/sippies (pre-filled--they will allow this through security, at least they have for us in the past)
4). food for her (the gerber yogurts that don't have to be refrigerated, cereal bars, puffs, etc)
5). Lots of hand wipes. I'm not a germaphobe but I know planes are pretty notorious for being super germy.
6). Tylenol/Mylecon (she's teething so there's kind of no living without it at the moment...)
7). Teething ring
8). Lovey/blanket
9). 2 sets of spare clothes, bibs, burp cloths, etc.
