I did tweak it a bit to fit my and my employer needs. Today is my first day on maternity leave though I'm due in 2 weeks. I needed time for my body and to take care of my home before baby gets here. The template gives different options. Good luck!
My office just had a really easy form to fill out for FMLA. Manager signed it and he knows my due date. He also knows I plan to work right up until I go into labor.
I had a ton of forms and then I got a letter from my OB:
To whom it may concern:
Mrs. X is under my prenatal care with an expected due date of November 7, 2008, barring any complications. If the patient has a normal delivery, the discharge date would be one to two days after delivery barring any complications. If delivered by cesarean section, the anticipated dishacrge date would be three to four days after delivery barring any complications. Should you have questions regarding this matter, please call my office.
