Update - SIRM phone consult

Just got off the phone with Dr. Brown. I have hope again! She went over absolutely everything with me. Whew!!! Here goes:

- I have to have another saline sono done to check my fibroids that are growing back.

- She told me to get off the DHEA. She said it can raise androgens & that can cause follies not to grow. (I didn't know that!!!)

- Told me she wanted me to do the antagon protocol again, but with BCP for less than 2 weeks. She thinks I was oversupressed by the BCP.(I was on BCP for 6 weeks on my first canceled cycle and over a month on the other canceled cycle)

- She said she didn't want to do anything about my hydrosalpinx because she's afraid of more scarring. They will drain it just prior to ET.

- She also said she wants to put me on intralipid therapy for my next cycle because my tumor was endometrial.

- I can do all my monitoring appts locally here and just to ER and ET in NY. (I already called the RE down the street from my house & they are willing to do all of this with us!)

- Wants me to do PGD and try to get to blast, instead of a 3dt. She said if it's not going to make it to 5dt, it would likely not implant anyway.

She also said that if this cycle doesn't work, that she would be willing to cycle with my sister's eggs, even though my sister is 41. She said we would def. do PGD with those embryos along with an amnio if I were to get pregnant.

Wow! I'm so glad I did this!!!

If all goes well, I may be cycling with them in March! (Just waiting for them to be accepted "in network" with BCBS. Hopefully in the next few weeks.) Woohoo!

  • Sounds very good!  I recall you having phone consults set up with a couple of other clinics.  Have you spoke with them yet?  Are you definitely moving ahead with SIRM? Just curious as I am a SIRM from the NJ location.

Did Dr. B think that the DHEA would be out of your symptom enough to not cause issues for a March cycle?  They say it takes 3 months to see the effects of DHEA so I am wondering if the effects are bad, does it take three months to leave your system.  Or, is the androgens only an issue in the couple of weeks leading up to your cycle?

    TTC #1 since 9/07
Dx MFI, AMA, Endo, AMH .16
Lap 10/09 Removed endometrioma, stage IV endo and adhesions
Lap 2/10 Endometrioma cysts & adhesions returned.

I didn't ask her. I haven't been doing the DHEA since October, so I don't think it's going to be an issue.

I think I may cancel my phone appt with CCRM. It's not until March, though, so I think I'm going to hold off for a little while before I do. I want to make sure I really don't want to do it.

I really am leaning toward cycling with SIRM. She seemed so much more knowledgeable than my current RE about DOR and the science behind it. My current RE is kind of like, "Well, we can try it!", but doesn't sit down and explain things out like she did.

  • hi cutie -

glad it went well :)  Dr. B is very sweet.

    i have a ? and suggestion

did she mention using Dr. Sher's EPP/Antagonist protocol for DOR at all? if she didn't ask if she thinks this might be a better fit

i would keep the consult with CCRM, especially if it's free.  i am all about getting as much info as possible

That's good, it's a non issue then.   I have to admit I stayed on the DHEA despite my Re's disapproval. I am planning to stop this weekend.  I start stims on monday.

This is why we chose SIRM too.   We met with a total of 5 RE's before making the decision and Dr. P was the only one who seemed to have science to back their treatment.  I assume the others clinics have tried and true methods as well, but at this stage of the process we needed a doctor who spoke to us as a equals.

    Hey, worms.

We did talk about the EPP protocol. That was similar to one that I had been on with my last cycle that was canceled. She thinks that the antagon protocol may be better suited for me.

Unfortunately, the CCRM consult isn't free. I have to pay $250 for it. Sad I'm going to hang onto it for now, but I may end up canceling it as it gets closer, depending on where we are with cycling at SIRM.

Thanks for all the great info you shared! I REALLY appreciate it!!! ::hugs::

  • Sounds like a great consult and a great plan!

Exactly! I feel like I know more about this process than most of their patients, I appreciate being given as much information as possible.

  • I'm glad you had a good meeting! It's interesting what Dr T and Dr B say about DHEA vs the CCRM approach. What are your thoughts about this?

To be honest, I'm over 40 and respond great. EQ is "as expected" by my RE and Dr T. It's not particularly encouraging to see so many nice embies poop out after d3. But as long as you go in with open eyes, it's great that you can cycle with your sister!

Good luck with clearing you insurance with them!

  • You will love SIRM! I am so glad I switched to this clinc for IVF #2.  Dr. B is really sweet.  She did one of my pre-cycle ultrasounds when Dr. T was out of the office.  I am glad you have the option of cycling with your sister's eggs.  Best Wishes Sweetie...
