Trouble TTC

BFFN again and a WWYD?

Oh well.  My hopes weren't up to high and I haven't even cried yet...

DH had 8 million total swimming sperm at his last SA.  RE would like to see 10 million before doing an IUI but will do one if we ask.  CD 1 should be on Friday and we also have DH's first urology appointment on Friday.

I don't know how to do a poll, but if you were me, would you go for the IUI or wait 3 months to try to get his numbers up?  There are no problems with me... would you try unmedicated first? or go for the Clomid to increase targets for his boys?

Baby N conceived after 1 miscarriage and more than 2 years of TTC. Diagnosis was low sperm count. We found success after 3 months of anastrozole to increase DH's testosterone and one IUI.

Re: BFFN again and a WWYD?

  • We're in a similar situation, except DH has ok-ish counts pre-wash (40 million) but post-wash only had 2 million. We did the IUI anyway, and are doing one more. If the post-wash numbers aren't better we're probably going to start looking at IVF, mainly because I'm 35 and I don't really want to waste my time and money with IUI's that are unlikely to work.

If we had known about his low numbers pre-IUI, I probably would have waited a few months to see what we could do to improve things. Better numbers will certainly improve your chances with IUI's and it's so hard to go through all this just to end up with a BFN. If you do decide to do IUI I would definitely do medicated. The more targets the better. Good luck! Hopefully the urologist will have some ideas for how to improve the counts!

Do you have IF coverage? Or if you have high co-pays, or only a few covered IUI treatments?

Our insurance covers 3 IUIs, so in my situation (if I were you) I would do Clomid with timed intercourse, while DH works on improving his numbers and then do the IUI when he had better numbers.

We have full IF coverage.  I know we can only do 4 IVFs, but I haven't heard about a limit on the number of IUI's.  I'll call my insurance now and ask... because that's a really good question!

  • Okay, my insurance called back and we are set for unlimited IUIs. (with a review after 3 to see if it still makes sense to continue.  but she made it sound like it would be easy to get continued coverage).

So, Mtooms, in that case, would you go ahead and do the Clomid + IUI now?

Also, we believe that his MFI is genetic, since his father had it too.  So, in that case, it's not like we can fix it.

  • I think I need to move to Illinois. You guys have the best IF insurance coverage!

I would go ahead with it since finances aren't going to be an issue. It's a bit more time consuming than doing things the old fashioned way, with monitoring, etc. but if you are emotionally ready to start treatment (that was what delayed us) and have the time it should improve your chances.

Come on to Illinois and become a Chicago Public schools teacher -- you will get awesome coverage.

  • I'd definitely do an IUI, but the Clomid is a little harder of a decision.  The reason I say IUI is that doing TI with those numbers is going to be an uphill battle for you.  The idea that it takes only 1 is crap- yes, only 1 fertilizes the egg, but it takes millions to protect that 1 along the way.  And when you think about the fact that only like 1% of sperm make it to your uterus and then another tiny percent make it anywhere near the egg.  We are dealing with MFI as well and to me, giving them a head start by doing IUI is the way to go.

We did 2 unmedicated cycles because I also ovulate on my own.  My RE wanted to do one more, but agreed to put me on Clomid this cycle.  He basically said that Clomid isn't the be all end all and can cause more problems than it helps and may actually make it harder to get pregnant depending on how your body reacts (dries up CM, thins out lining, causes cysts, etc).  So I agreed to give the natural cycles a try and when they didn't work, moved on to Clomid.

GL to you in whatever you decide :)
