I feel really lucky that I do, and it makes me sad that so many insurance providers do not provide IF benefits. I know if I didn't have them, we probably wouldn't be able to pay for an RE and all of this IUI business.
I just switched to my employer's insurance from DH's. Last year we had zero IF coverage so we paid OOP for all of our testing & our first (and only IUI). My current insurance has a $5k annual max for IF and it covers drugs and anything other than IVF. It did cover my monitoring for IVF, just not the "IVF" procedures, retrieval, transfer, embryologist fees, etc. With just that minimal coverage we've almost maxed out the $5k. I'm very grateful to have the coverage we have, even though it's minimal.

We do have coverage thru DH's job, but we have a $5,000 lifetime max. Up to that point they will pay 80% of everything. We have separate drug coverage as well. DH will be changing jobs (actually just changing employers) in April, so we'll have to wait & see what we have then.

I only have diagnostic coverage so my meds and IUIs are OOP. I was fortunate though that for this IUI I only had to pay OOP for one medicine. I think it was a fluke but I asked twice and then just paid for the 1.

I pay 50% for all IF services. My husband has the same carrier and the same coverage. 2 ultrasounds, 1 IUI and meds (femara, progesterone and trigger) is $500. So we'd be paying $1000 if we didn't have this coverage. The would really limit the number of IUI's we could do. I imagine we would only be able to do an IUI every other cycle.
We are 100% oop for everything--diagnostics, treatments, meds. DH's insurance has coverage for diagnostics, but the premium to add me to his policy was so much that it didn't make sense. His insurance is HSA compatible, so we've been able to pay for most things pre-tax, which actually is a fairly substantial savings tax-wise. I guess that's better than nothing. I am lucky that my clinic is affiliated with the university here, and their pharmacy gets meds through the state so that's not really costing us much. For my last cycle I only paid $14 for Clomid and $10 for the Ovidrel!

We are very lucky. Our insurance pays for 3 IUIs, with just co-payements. And the co-payment for IVF is 25%. Hopefully though the IUIs will work! I am very scared about IVF.
My insurance covers all testing and treatments 100% plus AI for up to $25k. I know that we are very lucky to have such great coverage. MH switched jobs in October last year and we kept his old insurance (COBRA), because the new job doesn't offer benefits and I am a temp, so benefits are crappy. We pay a lot of money every month for this insurance, but think it is worth it.

We have some coverage for testing, but absolutely no coverage for treatment or meds. So, basically, we don't have much. It does suck. I wish I had the ability to get IF coverage. Not having coverage may mean that we cannot financially continue in our quest to TTC. I hate that money may be the reason that we don't have a child. I also hate that even if we do have a child, we no longer have money saved as a cushion when we start having extra expenses.

Dx: DOR, Endo, APA+ (really high beta 2 glycoprotein antibody and high everything else tested), heterozygous MTHFR mutation, positive for lupus anticoagulant, high FSH, low AMH and both tubes blocked (per HSG on 3/8/11)
IVF #1 - long lupron (with HGH, intralipids, lovenox and BA); 4 retrieved, 3 fertilized; ET 2 blasts and 1 frozen = BFN
IVF #2 - a version of antagonist with EPP (with HGH, intralipids, lovenox and BA); 6 retrieved, 4 mature, 3 fertilized, 2 blasts and 1 frozen blast transferred on day 5 = BFN.
IVF #3 April was postponed to May, May was canceled. June/July was canceled. Had a cyst aspiration and then began IVF #3 in August. ER on 8/22; ET on 8/24 with AH. +HPT on 9/5. Beta #1 (11dpo) = 3; Beta #2 (15dpo) = 29; Beta #3 (17dpo) = 60; Beta #4 (19 dpo) = 118. Heartbeat at 6 weeks 6 days =132. Lil is here!
We have coverage for all testing, IUI's, and any IF drugs including injectibles, and I feel so so lucky to have that. We will be OOP for everything but meds if we need to do IVF, so I hope we don't get to that point.

My insurance covers up to a lifetime max of $15,000 which is pretty much one round of IVF. But we still have to pay 20% of pocket as well... for the procedure

After 6 years of failed cycles, we were blessed with our little man through adoption.
We are OOP for everything, also. It makes it so much harder. We are most likely doing IVF this next cycle, and I feel like it puts so much more pressure on this first cycle working.

I have IF coverage but I do not have IUI or IVF coverage. My coverage is through my employer and is pretty good. I am very grateful for the coverage that I have. All I really need to worry about is my deductible and copay. Since I had my surgery in the beginning of the year, my deductible is already met.

3/30/11 Biopsy results: cervial and endometrial pathologies normal; one pathology came back abnormal with 5 rare cells from the endometritis

We have a $500 deductible and we pay 10% after that up to a lifetime max of $20,000. I met the deductible in clomid round one so we're good to go. If we met the lifetime max, I think we can switch providers (dh's work offers two choices) and continue on.

I'm in NJ, a mandate state, and am covered 90% up to $25K lifetime, including testing for me and DH. Drugs are covered separately. I've already used up a few thousand the $25K for tests and doctor's visits.
While I am extremely grateful for the coverage I do have through my employer... if I told you where I worked, it would seem pretty pathetic (let's just say it's somewhere healthcare related).

We're 100% OOP. We did just find out that the cost of our meds can go toward the deductible, which helps a little. Like PP stated, I'm thankful we have money saved and can go forward with IVF.

Yikes, I'm so jealous! And I'm not even "in it" fully yet. We are covered for diagnostic testing only after $2500 deductible. Then nothing after that. We did all the diagnostic last year, so I'm back to paying the deductible if the RE wants to retest anything. Thankfully they work hand in hand with the OB's office so I think they will accept all my existing results.
We have 50% coverage for IF diagnostic, treatment and meds for anything up to an IUI only. GIFT, ZIFT and IVF-related items are not covered. So if we ever need to go to IVF, we're 100% OOP.
