We still don't know for sure if this IUI worked or not, but seeing that I've done 5 of them before this one with no noticeable differences I'm not holding out much hope. My nurse recommended we set up our IVF consult within this cycle so that if it doesn't work and we're ready, we can move forward with the testing next cycle and IVF the cycle after that.
So, here we go. I was told that the appt should take about an hour. I'm hoping the doctor explains everything so well that I don't have any questions. DH and I are trying to come up with some questions tonight though just in case. I feel like I don't even know WHAT to ask though. <sigh>
I'm incredibly scared to move on to IVF. I have no idea how I'm going to juggle work and an IVF cycle at the same time. I'm worried it won't even work and we'll find out my eggs are, indeed, crap when they are able to study them. I'm nervous about the meds, s/e, pain, etc etc etc. I know I need to take one day at a time but I'm freaking out a little!

So sorry for your IUIs not working. We were very nervous about our IVF appt and remembering to ask all the questions. I felt the same way - I didnt even know what to ask. Thankfully, our RE covered everything before I even pulled out my questions. I hope you have the same experience. And, if you forget anything, you can always call or email your nurse.
I was freaking out thinking about IVF too, and I just had to adopt the same mantra...one day at a time. Good luck!

Me= 37 and DH = 41
Dx: DOR, Endo, APA+ (really high beta 2 glycoprotein antibody and high everything else tested), heterozygous MTHFR mutation, positive for lupus anticoagulant, high FSH, low AMH and both tubes blocked (per HSG on 3/8/11)
IVF #1 - long lupron (with HGH, intralipids, lovenox and BA); 4 retrieved, 3 fertilized; ET 2 blasts and 1 frozen = BFN
IVF #2 - a version of antagonist with EPP (with HGH, intralipids, lovenox and BA); 6 retrieved, 4 mature, 3 fertilized, 2 blasts and 1 frozen blast transferred on day 5 = BFN.
IVF #3 April was postponed to May, May was canceled. June/July was canceled. Had a cyst aspiration and then began IVF #3 in August. ER on 8/22; ET on 8/24 with AH. +HPT on 9/5. Beta #1 (11dpo) = 3; Beta #2 (15dpo) = 29; Beta #3 (17dpo) = 60; Beta #4 (19 dpo) = 118. Heartbeat at 6 weeks 6 days =132. Lil is here!
TTC#2: Trigger + TI = BFN; Clomid + Trigger + IUI = BFN.
IVF #4: BCP + MDLF + Lovenox = 7R, 1F = Transferred 1 6-cell embryo on day 3 = BFN
IVF #5: MDLF + Lovenox = 4R, 1F = Transferred 1 10-cell compacting embryo on day 3 = BFN
IVF #6: (New RE): Long Antagonist November 2014 (transferred two 8 cell grade 1 embryos and froze one blast) = BFN

I am sure your dr will cover most everything when she goes over things. Here is the lost of questions I asked. I can't actually claim these questions as mine though - mdluv gave me her list and I just modified them a little bit to my circumstances. I also got some ideas from some other girls.
Do you think my partner and I are good candidates for IVF? Do you see any major obstacles to my getting pregnant via IVF?
What are your most recent live birth rates for women in my age group and couples with our condition?
What tests do we need to complete or have re-done before we can start a cycle? Can I start to cycle immediately (I am on cycle day 2 or 3 depending)
Based on my records and how I have responded to injectables do you know what kind of protocol you would be recommending I do?
How often do you start new cycles? How long do you generally keep a patient on BCP and Lupron?
Are there higher success rates with BCP?s and lupron as opposed to just lupron due to over suppression on lupron alone (longer for new cycle to start?
My cycles are on the longer side, normally 29-33 days, will Lupron make my cycle longer, thus I will be on it longer and end up being over suppressed?
Do you prefer a 3dt or 5dt? How few embryos do you have to have to do a 3dt? How many embryos do you usually recommend putting back in each of the two scenarios?
Does insurance go towards a shared risk program and how much additional would it be? Do you think I would qualify to participate in your shared risk program?
What is the criteria for triggering? Do aim for a certain amount of follicles? What circumstances would warrant canceling a cycle?
Will you be doing ICSI - Additional Costs? Is there a higher success rates? Increased risk of miscarriage or birth defects when ICSI is used?
Is assisted hatching something you think I need to consider? Additional costs?
What do you think the success rate based on what we have discussed would be? Of that what chance of multiples?
What are your thoughts on acupuncture increasing the success rates of IVF? I have already been going approx. weekly or every other for the last couple of months ? how often and when in my cycle should I be going?
Will I be able to begin a cycle right away? What is the basic timeline?
Whom shall we call when we have questions?
Are the fees for cryo due up front? What if there are no embryos to freeze, do we get our money back?
What procedure do you use for freezing remaining embryos? (Cryopreservation is the best method) What is the thawed vs/ fresh success rate? Where are the embryos stored? What are the storage fees?
What about PGD? Is that something we should consider given that we already had one loss due to chromosomal issues?
I also asked a few other questions specific to my clotting issues.
