bringing along my charts from the past 6 months and a list of vitamins and meds i've been taking even tho she should have it all but you never know 
anything else I should be thinking about? today we are checking out my prolactin level again and u/s and bloodwork(i think)

1/2012-S/A wash-count low 3 million
12/30/11 Luminarium(seaweed) stuck in cervix was suppose to "naturally" open it. Surgery for retrival.
12/7/11 Met with Dr. Dougherty new RE-more testing
2/11 Decided to take a year off to save for a new RE 
1/11 Cycle day 4 a/s 15mm cyst left Ovary 1 follicle left over no meds this month.
12/10 Cycle Clomid-O'd Progestrone Dostinex =BFN
11/10 HSG-ALL CLEAR & S/A-Adequate
10/10 Cycle- Progestrone and Clomid-O'd=bfn
11/09 lap-removed a lot of scar tissue/endo =not so much cramps during AF 

2006 Appendectomy-cyst burst on right ovary =scar tissue
2005 abnormal cells-biopsy and cells removed=scar tissue

Good luck at your appointment today. It sounds like you're more prepared than I was. When I went, I had just had a physical the week before including a pap, so when I went to the RE, I didn't bother to shave my legs or anything because I figured I wouldn't need to strip down again.... To my complete horror and embarrassment, the RE made me redo some of the tests...
Sounds like you've got it all covered, try not to be too nervous.
 
My RE required records from my GYN of my last pap smear and STD tests before he would move forward with any treatment. I didn't bring them the 1st meeting, but brought the records over the next week.
During the 1st appt, he just asked a lot of questions. Since I was CD7 he also did a baseline ultrasound (and predicted exactly the day I would O- 5 days later), along with some basic bloodwork.
IVF #1 = BFN
FET #1 = BFN
FET #2 = BFN
IVF #2 = BFP, b/g twins lost at 20w due to partial abruption/PPROM
IVF #3 = c/p 5w2d
Long-shot Clomid/Prednisone cycle before next IVF = BFP, our beautiful, healthy girl born 6/26/13!

FET #3 - May/June 2014 - all embryos arrested before xfer - back to the drawing board...

Good luck! When we went to our first RE appt, I got a pap smear, as it had been a year since my last one. I also got b/w, an u/s, and DH was scheduled for his SA. It lasted about an hour, and the RE spent some time explaining our "issues"
I felt very good leaving our appointment since we finally had a plan/hope.
