We are going to start LO on rice cereal. The doctor and the box say to mix 1 tablespoon of rice with formula or water....do you make a bottle first of formula (also a few tablespoons only, not ounces anymore?) and mix that with the rice, or just use water alone? I'm so nervous to do this wrong....

This exactly, you want it runny with no clumps.
First, don't panic. There is no right or wrong in making the cereal. If it's too thick, add more formula. If it's too thin, add more cereal. The tablespoon can be too much to eat for some babies and not enough for others so just do it as an approximate amount. It's not like formula where you should measure exactly.
I just make my bottle of formula I am going to feed dd and then pour a little into the bowl of cereal. If my dd is still thirsty and wants more formula, I can always make more but in the beginning of solids, you may find your LO only wants a few bites.
As for consistency- runnier than applesauce is a good description- for beginning. You can thicken it up after they get used to eating.
