I want to get a consent for medical treatment form notarized for my sitter in case GOD FORBID anything happen to Ashton and she needs to take him to the ER. His Ped said that I just have to add the sitters name to their form but that the hospital requires a notarized copy. Has anyone done this? Any ideas of what exactly the form should say? How do I include the Ins info or should I just give her an extra ID card for our poilcy?
TYIA for your answers and help....
Re: ok for Serious now... Medical Release Forms help
I found some good info here:

That's a good question. And maybe the Notary Public could let you know if anything else needs to be on the form, if he/she deals with this sort of form a lot.
Sadly, FIL got sick with a cold or something and couldn't make it up here.
Luckily, Joey is off today, but starting tomorrow, my parents are going to have to watch Torin. They've made it clear that they don't want to do it, but I don't have a choice. Grrrr....
But yes, I am checking on him constantly. Joey has sent me a few pics to tide me over till I get home. At the same time, though, it's nice to be able to bump pretty much limitlessly.
So will FIL come when he feels better? WOW that sucks. I know that your parents were never part of the equation so I can imagine how much chaos this is causing. Let me know if you need to vent. I'll bring the whiskey and cigars!
I think he will come up here after he feels better. I hope, I hope, I hope. Until then we'll just sip and puff our worries away. Ha ha!
