Preterm labor advice needed repost from high risk and preemies

So here's my story in a nutshell. At my anatomy scan I was diagnosed with a shortened cervix (1.8cm), sent to mfm at the closest facility to my home and was told I was not a candidate for cerclage being 20 weeks with twins. 2 weeks later I ended up admitted as I had shortened to a 1.1, was told again that they wouldn't do anything but bed rest and sent home. 2 weeks ago ( at 24 weeks ga) I was admitted to my local hospital due to being .6, 2cm dilated, and contractions. Procardia held them off for a while but 2 days ago it was discovered that I am now at a 4 with bulging membranes. I'm now at a facility with a great nicu, I decided I would rather go further away then get the dismissive care I had received before. I have had all 4 steroid shots, mag drip, and am on indomethacin. Phew, after all of that I'm just wondering if anyone can relate, and if so how long were you able to keep your babies cooking?( mine are26 5 days). I know every pregnancy is different but I'm having a hard time finding any stories even similar to mine. I do not have pre-e, my body just seems to think it's done. Thanks if you made it through all this especially with formatting issues, paragraphs are not the ipad's friend! Any advice you have would be greatly appreciated! All of you have been such fantasic resources during my pregnancy, TIA!

  • I was not in your exact position, but they did find my cervix shortening around 21-22 weeks, and I was also told they would not/could not do a cerclage at that point.  Bed rest kept me stable for the most part, although at 27 weeks my cervix went down to .6 and I went on strict bed rest.  I was able to go 9 more weeks.  I started my blog about a year ago to detail my bed rest, the link is in my signature.

    It is definitely scary to be dilated so early in the pregnancy, but one of my OBs (who is an MFM) told me that they had a patient who dilated at 18 weeks and was able to go to 37 weeks with her pregnancy.  So just stay put, and hope that you are stuck in the hospital for 9 more weeks!

    Keep us updated!  I spent a lot of time on the High-risk board while I was on bed rest and loved the support.

  • no advice or experience- but it sounds like you are in the best place for you and the babies right now.

    www.keepemcookin.com has lots of great info on PTL, treatment, etc. It's the most comprehensive spot for this kind of info I have seen.

  • I am in your position right now. I'm 24 weeks with a 1.3cm cervix.  I actually had an appt. last week and my cervix was  at 1.9cm, so its been shortening rapidly. I have an appt. next week to see if I will be admitted until the boys are born. They put me on progesterone, but I only started that this week. I have no problems or anything,  no contractions, no dialating, but its like you said, my body just thinks its almost done. The doctors have already given me steriod shots for the boys, so that has me thinking that they don't think they'll be in for a while, but it could just be better safe than sorry. The hospital that I'm delivering at is top rated in the country, so I'm not overly worried, just not looking forward them telling me that I have to spend the rest of my pregnancy there rather than at home.

  • I was in that exact same situation at that same time.  I had a 0.8 cervix at 20 weeks.  I was on strict bed rest from week 20 and made it to week 30.  I had two bouts of PTL, one at week 26 (which was stopped with the mag) and the other at week 30 which was not stopped.  I had two rounds of steriods between those weeks.  

     My girls are now one year old and perfectly healthy.

     I know its difficult, but try to take care of yourself, listen to your doctors, and REST. Hang in there.

  • Your story sounds very similar to mine. I went into PTL at 22wks5days where it was discovered that my cervix was at a 0.8. I too, was not a candidate for a cerclage since I was contracting too much and the risks outweighed the benefits at that point.

    I was on hospital bedrest for 10 weeks and was on a continuous magnesium sulfate drip and would receive indomethicin for breakthrough contractions. I was able to keep them in until 32wks1day. Hopefully, your uterus isn't as touchy as my uterus, because I couldn't lay on my back without going into strong contractions and was not able to sit up (even to eat!) because they were so worried about my cervix shorterning further.

    Just take it easy and rest. If something feels wrong, tell your doctor ASAP. Good luck!

  • My cervix was long & locked up tight. At 25w1d, I noticed rhythmic tightening in my belly & was told to head to L&D. I was contracting every two minutes & was barely dilated for 4 hours, whenever gave me a terbutaline shot. Ctx stopped, & they thought they'd send me home. Checked my cervix again, & I was 2 cm. Ctx started again, I was admitted, given a rushed dose of steroids, started a mag drip & was dosed with indomethacin. Seven hours later, I was still contracting, so they started me on every-2-hr terb shots, which finally stopped all ctx. I was weaned off all 3 meds 36 hours after admission & stayed in the hospital an additional 2 nights for observation. I was released on home-bedrest orders with as-needed terb pills at 26w1d. At 28w, I noticed the same rhythmic tightening & took terb. Ctx stopped, so I didn't call (told to call if they kept up) but intended to dose through the night until my MFM appt the next morning. An hour before my third dose, I woke to pee, stood and felt a warm gush. My water broke. I delivered my triplets 4 hours later via emergency c-section.

    Follow orders. I did, and I have the comfort of knowing I did my best to keep them in. I wish I could have kept them in longer, but I know we all did our best for them, and that's so important. Good luck cooking, and try to keep your chin up! Drugs and bedrest suck, but they may be lifesavers!
